摘　　要：目的评价颧弓上缘小切口入路治疗颧弓骨折的临床价值。方法收集2014年6月至2017年5月河南科技大学第一附属医院口腔颌面外科收治的38例颧弓骨折,颧弓二线骨折者12例,颧弓三线骨折者23例,颧弓粉碎性骨折者3例。所有患者均在全麻下使用颧弓上缘小切口入路行颧弓骨折切开复位内固定手术。术后随访0.5 a,复查CT,分4级对治疗效果进行评定。结果所有患者切口均Ⅰ期愈合,术后未见面神经受损症状。38例患者完成术后随访者34例(89.47%)。术后0.5 a随访时,局部伤口未见明显手术瘢痕。治疗效果Ⅰ级者30例(88.24%),Ⅱ级者4例(11.76%),无Ⅲ级及Ⅳ级者。结论颧弓上缘小切口入路治疗颧弓骨折无明显手术并发症,是治疗颧弓骨折的较佳手术入路,值得临床推广应用。Objective To evaluate the clinical value of the micro-incision approach on superior margin of zygomatic arch in treatment of zygomatic arch fracture. Methods From June 2014 to May 2017,38 patients with zygomatic arch fractures were treated with open reduction and rigid internal fixation by micro-incision approach superior margin of zygomatic arch under the general anesthesia from the First Affiliated Hospital of Henan University of Science and Technology.Half year after operation,the CT image was reexamined and therapeutic effect was also evaluated at 4 levels. Results All incisions were healed in stage I,with no symptom of facial nerve hurting.Thirty-four of the 38 patients(89.47%)completed the follow-up.During the 0.5 a follow-up,there was no obvious surgical scar.Thirty of the 34 patients(88.24%)was healed in levelⅠ,4 patients(11.76%)was healed in levelⅡ.No patient was healed in levelⅢor levelⅣ. Conclusion The micro-incision approach superior margin of zygomatic arch is a great choice for the treatment of zygomatic arch fracture in inconspicuous complication,and it is worthy of clinical popularization and application.
