基于优质医疗服务理念下住院医师培养的思考

摘　　要：目的对比不同来源一线住院医师所管理的外科手术后患者满意度及术后短期的生活质量,并分析其影响因素,为提供优质医疗服务理念下住院医师培养提供参考。方法将本院一线住院医师分为三类:本院职工一线及外院高资历进修医师(简称高资历医师)、本院专业型硕士研究生(简称专硕)、社会规范化培训医师(简称规培医师)。所有病例均为来自同一临床治疗组并分别由以上三类医师作为住院医师所管理的结直肠癌术后患者。采用自行设计的问卷及引用EORTC QLQ-C30,对2014年1月至2017年10月在我院普通外科住院符合条件的176例患者进行了满意度问卷调查及生活质量评估,问卷回收率为90.3%。结果在患者整体就诊满意度上专硕医师组优于其他两组医师,在短期生活质量上三组患者总体均较高,但高资历医师组有优于其他两组的趋势,而规培医师组介于二者之间。结论医学是一门实践性很强的学科,高资历医师实践经验丰富,临床问题识别处理能力较强,但同患者沟通耐性不足,因此病患术后整体生活质量较高,但满意度偏低。专硕医师专业理论知识牢靠、医患关系单一,具有极高的职业兴趣和对医学未来的憧憬,其与病患沟通充分。因此在住院医师培养过程中,既要重视专业能力的培养,还要强调对患者人文关怀的理念,这样才能给患者提供更加优的医疗服务,有利于我国医学及医患关系向健康稳定的方向发展。Objective To compare the satisfaction and the difference of quality life in short-term of the postoperative patients which managed by first-line residents of different origins,and analyze the influencing factors,so as to provide a reference for the cultivation of resident physicians based on the concept of high quality medical service.Methods The residents physicians in our hospital were divided into three categories:first-line staff in our hospital and other hospital senior qualifications,social standardized training physicians in our hospital and professional master.All cases were colorectal cancer patients from the same clinical treatment group and were managed by the above three types of residents physicians.A self-designed questionnaire and EORTC QLQ-C30 were used to conduct a satisfaction survey and quality of life assessment on 176 eligible patients in our hospital from January 2014 to October 2017.The questionnaire recovery rate was 90.3%.Results The patient satisfaction of the professional master group was superior to that of the other two groups,the three groups of patients were generally higher in short-term quality of life,but the higher-qualified physician group had better trend than the other two groups,the social standardized training physicians group was between the other two groups.Conclusion Medicine is a highly practical subject.Physicians with high qualifications are experienced in practice and have strong ability to recognize and process clinical problems,but have poor communication with patients.The overall postoperative quality of life of the patients is higher,but the satisfaction is worse low.Professional master has professional theoretical knowledge,a single doctor-patient relationship,high professional interest and the future on medicine,and commuicate full with the patient.In the process of resident physicians training,we must attach importance to the cultivation of professional ability,but also emphasize the concept of humane care for patients in order to provide patients with more ex
