MMSE正常的帕金森病患者定量脑电图与早期认知障碍的相关性

摘　　要：目的探讨简易神经状态量表(MMSE)正常帕金森病(Parkinson disease,PD)患者定量脑电图(quantitative EEG,qEEG)检查结果与神经心理测试量表评分的相关性。方法选取作者医院就诊的MMSE评分≥25分的PD患者65例,以及年龄、性别构成等匹配的健康者30名为对照组。运用qEEG技术检查各组不同脑区功率谱比值的变化,采用成套神经心理学量表测试各组认知障碍情况,采用Spearman相关性分析评估PD患者神经认知与qEEG结果的相关性。结果 PD组额叶执行功能、言语记忆形成过程、注意力持续时间、视空间功能的评分成绩低于健康对照组(P<0.05)。qEEG分析显示PD患者额、中央、顶、枕、颞区功率谱比值较健康对照组降低(P<0.01)。PD患者脑区功率谱比值与多项神经心理学测试评分呈中度正相关(r=0.406~0.672,P<0.05),而这种相关性在额区和颞区表现尤为密切。结论 MMSE正常PD患者qEEG结果和成套神经心理测试评分间存在中度正相关,qEEG检查对发现无明显认知障碍的PD患者早期认知损害可能具有一定的临床意义。Objective To investigate the correlation of quantitative EEG(qEEG)and neuropsychological assessments in Parkinson s disease(PD)patients with normal mini-mental state examination(MMSE)score.Methods Sixty-five patients with PD and 30 age-matched healthy controls were recruited in the study.All the participants had a MMSE score greater than or equal to 25.qEEG analysis and an extensive neuropsychological assessment battery were applied to all the participants.Linear correlation between the neurocognitive tests and qEEG analyses of the patients with PD was assessed using Spearman correlation analysis.Results Test scores for frontal executive functions,memory processes,attention,and visual spatial perception were significantly lower in the PD patients than the healthy controls(P<0.05).qEEG analysis revealed that the power spectral ratios(PSRs)([alpha+beta]/[delta+theta])in the frontal,central,temporal,parietal,and occipital regions were significantly decreased in the PD patients compared with the controls.The decline in PSRs was moderately correlated with most items of the extensive neuropsychological assessments battery(r=0.406-0.672,P<0.05).Especially,the correlations of qEEG analysis and extensive neuropsychological assessments battery were mostly occurred in the frontal and temporal region.Conclusions In patients with PD with normal MMSE,qEEG analysis is moderately correlated with extensive neuropsychological assessments.qEEG analysis may be a useful diagnostic tool in detecting early subtle cognitive dysfunction in PD patients.
