摘　　要：目的探讨TOAST分型与急性脑梗死(ACI)患者近期预后的关系。方法分析286例ACI患者的临床资料。比较TOAST分型各亚型患者的基线资料、入院时及入院1个月时NIHSS评分与NIHSS评分好转率、致残率、致死率及复发率。分析显效与无效患者的各临床指标及TOAST分型对ACI患者预后的预测结果。结果 TOAST各亚型间基线资料无显著差异(P>0.05)。TOAST各亚型患者入院时、入院后1个月的NIHSS评分及其好转率有显著差异(P<0.05)。ACI显效与无效患者的年龄、高血压史、TOAST分型及高纤维蛋白原血症有显著差异(P<0.05)。TOAST分型对ACI患者预后的敏感度为82.2%,特异度为53.4%。TOAST分型中LAA亚型、CE亚型的致残率及致死率、复发率均明显高于SAO亚型(P<0.05)。结论TOAST分型对ACI患者的预后有一定的预测价值。Objective To retrospectively study relationship between TOAST classification and short-term prognosis of acute cerebral infarction(ACI)patients.Methods Clinical data of 286 patients with ACI in our hospital was retrospectively analyzed.The general information,the NIHSS scores and its improvement rates,morbidity,mortality and relapse rates of different subtypes on admission and after one month of admission were compared.The effect of clinical indicators and TOAST classification of effective patients and invalid patients on the prognosis of ACI patients were analyzed.Results The general information in different subtype groups had no statistically significant differences(P>0.05).The NIHSS scores and its improvement rate on admission and after one month had statistically significant differences(P<0.05).The age,hypertension history,TOAST classification,high blood fibrinogen factors between the effective and invalid patients had statistically significant differences(P<0.05).The sensitivity and specificity of TOAST classification on the prognosis of ACI patients were 82.2%and 53.4%.The CE group and LAA group had higher disability rates,fatality rates,recurrence rates than SAO group.Conclusion The TOAST classification is of great predictive value on the prognosis of ACI patients.
