结肠、直肠恶性梗阻患者支架置入后择期术的临床分析

摘　　要：目的比较支架置入后择期术与急诊术在结肠、直肠恶性梗阻患者中的临床疗效。方法分析120例结肠、直肠恶性梗阻患者的临床资料。随机分成观察组(支架置入后择期手术)和对照组(急诊手术)。比较2组患者的一般资料、手术相关指标、手术情况及术后并发症发生率。结果 2组患者的一般资料无显著差异(P>0.05)。观察组患者手术时间、术后通气时间、术后并发症发生率均显著优于对照组(P<0.05),2组患者的手术情况也有显著差异(P<0.05)。观察组内行剖腹术者住院时间显著长于行腹腔镜手术者,而手术时间、并发症发生率显著低于腹腔镜术者(P<0.05)。结论与急诊术相比,支架置入后择期术治疗结肠、直肠恶性梗阻者的临床疗效显著,安全性好。Objective To compare the clinical efficacy of elective surgery after stent placement and emergency operation in treating patients with malignant colorectal obstruction.Methods Clinical materials of 120 patients with malignant colorectal obstruction were analyzed.They were randomly divided into observe group(elective surgery after stent placement)and control group(emergency operation).The general information,the operation related indexes,the operation situation,and the incidence rate of postoperative complications were compared between two groups.Results The general information showed no significant difference between two groups(P>0.05).The operation time,the postoperative ventilation time,the incidence rate of postoperative complications in observation group were significantly better than those in control group(P<0.05).The operation situation showed significant difference between two groups(P<0.05).In the observation group,the hospital stay of patients underwent laparotomy was significantly longer than that of patients underwent laparoscopic surgery,but the operation time and the incidence rate of complications were significantly shorterand lower than patients underwent laparoscopic surgery(P<0.05).Conclusion Compared with the emergency operation,the clinical efficacy of the electivesurgery after stent placement is effective and safety.
