温阳行气中药灌肠联合普芦卡必利治疗慢性功能性出口梗阻型便秘疗效及对肛门直肠测压、氧化应激指标的影响

摘　　要：目的观察温阳行气中药灌肠联合普芦卡必利治疗慢性功能性出口梗阻型便秘疗效及对结肠传输试验积分、肛门直肠测压、氧化应激指标的影响。方法将130例慢性功能性出口梗阻型便秘患者随机分为2组,对照组65例给予普芦卡必利口服,观察组65例在此基础上加用温阳行气中药灌肠治疗,2组疗程均为4周。观察2组治疗前后中医证候积分、SF-36评分、结肠传输试验积分、肛门直肠测压、超氧化物歧化酶(SOD)及丙二醛(MDA)水平,统计2组临床疗效。结果治疗后2组面色淡白无华、头晕眼花、心悸、失眠健忘、舌淡白苔薄及脉沉细积分、结肠传输试验积分、肛管静息压、肛管最大收缩压、持续挤压时间及MDA水平均显著降低(P均<0.05),2组生理功能、生理职能、情感职能、活力、社会功能、躯体疼痛及精神健康评分和SOD水平均显著提高(P均<0.05),且观察组上述指标改善情况均显著优于对照组(P均<0.05);观察组治疗总有效率显著高于对照组(P<0.05)。结论温阳行气中药灌肠联合普芦卡必利治疗慢性功能性出口梗阻型便秘可有效减轻症状体征,提高日常生活质量,改善结肠传输功能和肛门直肠压力,调节氧化应激指标水平。Objective It is to observe the curative effect of enema of Wenyang Xingqi drugs combined with propafarvilli on chronic functional outlet obstruction type constipation and its effect on colonic transit test score,anorectal manometry and oxidative stress.Methods 130 cases of chronic functional obstruction type constipation were randomly divided into two groups,65 cases in the control group were treated with propafasil,and 65 cases in the observation group were treated with enema of Wenyang Xingqi drugs based on the therapy in the control group.Both groups were treated for 4 weeks.The TCM syndrome score,SF-36 score,colon transmission test score,anorectal manometry,SOD and MDA levels before and after treatment were observed,and the clinical curative effects were evlauted in the two groups.Results After the treatment,the scores of pale and white face,dizziness,heart palpitations,insomnia and forgetfulness,thin tongue and thin veins,colon transmission test scores,resting pressure of the anal canal,maximum systolic pressure of anal canal and the level of MDA were significantly decreased(P<0.05),the scores of physiological function,physiological function,emotional function,vitality,social function,physical pain,mental health score and SOD level were significantly increased(P<0.05),and the improvements of the above indexes in the observation group were significantly better than those in the control group(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Enema of Wenyang Xingqi drugs combined with propafarvilli can effectively alleviate the symptoms and signs,improve the quality of daily life,improve colonic transit function and anorectal pressure,and regulate the level of oxidative stress in the treatment of chronic functional outlet obstruction type constipation.
