经乳晕弧形切口切除术治疗乳房纤维瘤对乳房损伤及术后乳房外观满意率的影响

摘　　要：目的:分析经乳晕弧形切口切除术治疗乳房纤维瘤对病人乳房损伤及术后哺乳率的影响。方法:选取60例乳腺纤维瘤病人,依照随机数字表法分为观察组及对照组,观察组选择经乳晕弧形切口切除术治疗,对照组选择以乳头为中心的常规手术方式做放射状切口治疗,观察2组病人手术过程中的出血量、手术时间、术后拆线时间、瘢痕大小、血肿面积大小、病人对术后乳房外观的满意度、生育情况以及生育病人的哺乳情况。结果:观察组病人手术时间明显长于对照组(P<0.01),但观察组病人术中出血量明显少于对照组(P<0.01),术后拆线时间明显短于对照组(P<0.01);观察组病人瘢痕长度明显少于对照组(P<0.01);2组病人术后各并发症发生率差异无统计学意义(P>0.05);观察组病人的乳房外形美观满意率明显优于对照组(P<0.01);观察组病人再生育的哺乳率与对照组差异无统计学意义(P>0.05)。结论:经乳晕弧形切口切除术治疗乳房纤维瘤病人可有效降低病人的术中损伤,提高病人对乳房外形美观满意率。Objective:To analyze the effects of the resection by areola arc incision in treating breast fibroma on breast injury and postoperative breastfeeding rate.Methods:Sixty patients with breast fibroma were randomly divided into the observation group and control group.The observation group was treated with the resection by areola arc incision,and the control group was treated with the conventional radial incision by nipple as the center of operation.The peroperative bleeding,operation time,time of postoperative stitches removal,scar size,hematoma size,satisfaction of postoperative breast appearance,fertility and breastfeeding of fertility patients between two groups were compared.Results:The operation time in observation group was significantly longer than that in control group(P<0.01),but the peroperative bleeding and time of postoperative stitches removal in observation group were significantly less than those in control group(P<0.01).The scar length in observation group was significantly less than that in control group(P<0.01),and the difference of the incidence rate of complication between two groups was not statistically significant(P>0.05).The satisfaction rate of postoperative breast appearance in observation group was significantly better than that in control group(P<0.01),and the difference of the breastfeeding rate of fertility patients between two groups was not statistically significant(P>0.05).Conclusions:The resection by areolar incision in the treatment of breast fibroma can effectively reduce the peroperative injury,and improve the postoperative breastfeeding rate of patients.
