黄芪三七汤对心源性脑栓塞致MMP-9升高及其出血转化的临床研究

摘　　要：目的:分析黄芪三七汤对心源性脑栓塞患者MMP-9表达的影响,探讨该药物用于防治心源性脑栓塞出血转化的可行性。方法:将我院2017年3月-2018年7月期间收治的120例心源性脑栓塞患者随机分为治疗组和对照组,两组患者均实施常规治疗,治疗组患者在常规治疗的基础上给予黄芪三七汤治疗。结果:两组入院24h内的血浆MMP-9检测结果、NIHSS评分、脑出血转化评分比较均无明显差异(P>0.05),治疗后7d、14d治疗组患者血浆MMP-9水平、NIHSS评分、脑出血转化评分均低于对照组患者(P<0.05)。治疗组患者的脑出血转化发生率为15.0%,低于对照组患者的28.3%、(P<0.05)。结论:黄芪三七汤能够抑制心源性脑栓塞患者血浆中MMP-9的高表达,降低患者脑出血转化发生率,用于防治心源性脑栓塞出血转化具有可行性。Objective:To analyze the effect of the Huangqi Sanqi decoction(黄芪三七汤)on the expression of MMP-9 in patients with cardiogenic cerebral embolism and the feasibility on hemorrhagic transformation.Methods:120 cases were randomly divided into the treatment group and control group.Both groups were treated with routine treatment.The treatment group was treated with the Huangqi Sanqi decoction more.Results:After 7d,14d of treatment,the MMP-9,NIHSS score,cerebral hemorrhage transformation score in the treatment group were lower than those in the control group(P<0.05).The rate of cerebral hemorrhage conversion in the treatment group was 15.0%,which was lower than 28.3%in the control group(P<0.05).Conclusion:The Huangqi Sanqi decoction can inhibit the high expression of MMP-9,reduce the incidence of intracerebral hemorrhage transformation,so it is feasible for preventing and treating hemorrhagic transformation of cardiogenic cerebral embolism.
