高频彩色多普勒超声诊断外周神经源性肿瘤的价值

摘　　要：目的探讨高频彩色多普勒超声对外周神经源性肿瘤的临床诊断价值。方法分析研究该院2012年1月—2016年3月经手术或穿刺病理证实的70例外周神经源性肿瘤患者的超声表现,观察肿瘤的大小、形态、边界、内部回声、血流信号,以及重点观察肿瘤与神经干的关系。结果外周神经源性肿瘤患者,声像图表现以椭圆形、梭形为主,边界清晰,有完整清晰的包膜或假包膜。神经鞘瘤部分肿瘤内部回声均匀,部分出现液化。神经纤维瘤及神经纤维瘤病的肿瘤内部回声均匀,均无液化。CDFI显示大部分肿瘤内部及周边可见血流信号,且以I^II级血流为主。外周神经源性肿瘤特征性表现是肿瘤两端或一端或其旁可见条状低回声的神经干相连,该研究中28例可发现此征象,其中位于四肢22例(占78.6%)。结论外周神经源性肿瘤具有一定的特征性声像图改变,特别是对于四肢部位的神经源性肿瘤,高频彩色多普勒超声可以清晰地观察到肿瘤与神经干的关系。术前超声明确诊断对临床手术成功治愈且不留后遗症具有重要的指导作用。Objective This paper tries to study the diagnostic value of high-frequency color doppler ultrasonography in peripheral neurogenic tumor.Methods 70 patients with peripheral neurogenic tumor with ultrasonic m anifestations confirmed by the pathological exam ination after surgery or puncture in this hospital from January 2012 and M arch 2016 were analyzed.The size,shape,boundary,internal echo and blood flow signals of the tumor,especially the relationship betw een the tumor and the nerve trunk were observed.Results As shown in the ultrasonogram,the peripheral neurogenic tumor generally had oval and spindle shape,with clear boundary,com plete and clear capsule or pseudocapsule.Some neurilem m om a had homogeneous internal echo,and the others were liquefied.Neurofibrom a and neurofibrom atosis had homogeneous internal echo,without signs of liquidation.CDFI indicated blood flow signals,m ainly were grade I-II,can be seen inside and surrounding the tumor.The distinctive feature of peripheral neurogenic tum or was connection of the tum or to the nerve trunk at both ends,one end,or the adjacency of the tumor,which was displayed by the strip hypoecho.In the present study,this sign can be found in 28 cases,among which the lim bs were affected in 22 cases(accounting for 78.6%).Conclusion Peripheral neurogenic tum or has distinctive features in the ultrasonogram.The relationship betw een the tumor and the nerve trunk,especially for neurogenic tum or in the lim bs can be clearly observed with high-frequency color doppler ultrasonography.Definite ultrasonic diagnosis before surgery is very im portant for successful treatm ent w ithout sequela.
