青光眼患者采取复合式小梁切除术联合玻璃体注药的疗效观察

摘　　要：目的分析对新生血管性青光眼患者予以玻璃体腔注射雷珠单抗、复合式小梁切除术联合治疗的应用效果。方法将本院2016年6月~2017年6月收治的36例(36眼)新生血管性青光眼患者进行观察,根据就诊先后顺序将患者分为联合组、参照组各18例。参照组给予复合式小梁切除术单独治疗,联合组予以玻璃体腔注射雷珠单抗及复合式小梁切除术治疗。观察并记录联合组患者行玻璃体注药后虹膜新生血管消退时间、眼压情况以及复合式小梁切除后两组患者眼压、视力数据。结果联合组患者在注射药物后,虹膜新生血管完全消退时间为(6.2±2.6)d;联合组术后1天、7天、1月、3月的眼压较之前显著降低且幅度优于参照组(P<0.05);比较两组并发症发生率无统计学意义(P>0.05)。结论玻璃体腔注射雷珠单抗及复合式小梁切除联合治疗能显著消退虹膜新生血管,控制眼压,疗效安全可靠。Objective To Analyze the effect of intravitreal injection of ranibizumab combined with trabeculectomy in the treatment of neovascular glaucoma.Methods 36 cases(36 eyes)of neovascular glaucoma admitted to our hospital from June 2016 to June-2017 were observed.The patients were divided into combined group and reference group by 18 cases.The combined trabeculectomy was given by the reference group,and the combined group was treated with ranibizumab and compound trabeculectomy.The time of iris neovascularization regression and intraocular pressure change and the intraocular pressure and visual acuity of two groups after combined trabeculectomy were observed and recorded.Results After treatment,the patients in the drug injection,iris neovascularization disappeared completely in time for(6.2±2.6)d;the combined group after 1 day,7 days,1 month,3 months,the intraocular pressure was improved significantly and improve the situation better than the reference group(P<0.05);two complication rate comparison there was no statistically significant difference(P>0.05).Conclusion Intravitreal injection of ranibizumab combined with trabeculectomy can rapidly reduce the atrophy of iris neovascularization,control intraocular pressure.It is safe and reliable.
