非肌电图下肉毒毒素治疗先天性内斜视的观察与护理

摘　　要：目的观察非肌电图下肉毒毒素治疗先天性内斜视的临床疗效与相关护理措施,以探讨其作为斜视手术替代疗法的可行性。方法对2015年3月至2016年6月在我院确诊的20例先天性内斜视儿童患者应用直视Tenon囊下注射A型肉毒毒素进行治疗与护理,每次注射2.5u。患儿平均年龄为4.23±1.31岁,男性11例,女性9例。对患者进行注射前后护理并观察其视力、屈光度、斜视度、双眼视觉功能等项目,并在治疗后1周、2周、1个月、2个月、3个月、6个月后对患儿进行随访,以斜视度≤10△作为正位标准。结果 20例患儿治疗6个月后总治愈率为75.00%(15例),而治疗3个月后的治愈率为55.00%(11例)。年龄<3岁的患儿治愈率为77.78%,3~6岁的患儿治愈率为71.43%,>6岁的患儿治愈率为75.00%。周密的护理使患儿家属对整个治疗过程配合好。结论 非肌电图下采用肉毒毒素对先天性内斜视患儿进行治疗的疗效良好。患儿年龄为1.5~3岁治疗效果更好。详细专业化的护理为治疗成功的保障。Objective To investigate the clinical efficacy and nursing measurements of botulinum toxin in the treatment of congenital esotropia without Electromyograph.Methods From March 2015 to June 2016,20 patients with congenital esotropia were treated with direct injection of botulinum toxin A in our hospital,each injection was 2.5 u.The average age of children was 4.23±1.31 years old,including 11 boys and 9 girls.Visual acuity,diopter,strabismus,binocular visual function and other projects of children were observed after the injection of botulinum toxin type A,the follow-up periods were in 1 weeks,2 weeks,1 months,2 months,3 months and 6 months after treatment.The degree of strabismus was≤10△as a standard.Results The cure rate of 20 cases after treatment for 6 months was observed,the total cure rate was 75.00%(n=15),and the cure rate after treatment for 3 months was 55.00%(n=11).The cure rate of children aged<3 years was 77.78%,aged 3~6 years was 71.43%,and aged>6 was 75.00%.Patients were totally cooperated due to comprehensive nursing.Conclusion The application of botulinum toxin in the treatment of congenital esotropia in children without electromyograph had good curative effects.The treatment effects are better in children with 1.5~3 years old,the detailed and specific nursing are essential.
