口腔种植修复技术对慢性牙周炎患者种植体松动度、PD及SBI指数影响研究

摘　　要：目的:探究口腔种植修复技术对慢性牙周炎患者松动度、牙周探诊深度(PD)及龈沟出血指数(SBI)的影响。方法:选取慢性牙周炎牙列缺失、缺损患者80例(102枚种植体)作为观察组,牙周健康的牙列缺失、缺损患者80例(100枚种植体)作为对照组,两组患者均采用口腔种植修复技术。比较两组患者种植体留存率以及松动度,并比较两组患者术后6个月牙龈乳头指数(PIS)、改良菌斑指数(mPLI)、改良龈沟出血指数(MSBI)以及PD,采用酶联免疫吸附试验(ELISA)检测两组患者治疗前后龈沟液白细胞介素2(IL-2)、白细胞介素8(IL-8)水平。结果:两组患者术后6个月种植体松动度均为0,观察组种植体留存率97.06%,与对照组(100%)比较差异无统计学意义(P>0.05);观察组患者PIS、mPLI、MSBI以及PD与对照组比较差异无统计学意义(P>0.05);观察组患者术前、术后龈沟液IL-2、IL-8水平显著高于对照组,差异有统计学意义(P<0.05),且两组患者术后与术前比较差异无统计学意义(P>0.05)。结论:口腔种植修复技术对慢性牙周炎牙列缺损、缺失患者具有良好的近期疗效。PIS、mPLI、MSBI以及PD等较为优良,且不会加重种植体周围炎症。Objective:To investigate the effect of dental implant repair technique on the degree of relaxation,PD and SBI index in patients with chronic periodontitis.Methods:80 patients with chronic periodontitis and dentition defect treated(102 implants)and 80 patients with periodontal health and dentition defect(100 implants)were chosen as the object.The former was used as the observation group and the latter as the control group,then the treatment process was analyzed retrospectively.The implant retention rate and the degree of dental loosening were compared between the two groups,the gingival papilla index(PIS),the plaque index(mPLI),the modified gingival bleeding index(MSBI)and the depth of periodontal probing(PD)of 6 months after the operation were compared between the two groups,Enzyme linked immunosorbent assay(ELISA)was adopted to detect the levels of interleukin 2(IL-2)and interleukin 8(IL-8)in gingival crevicular fluid before and after treatment.Results:The implant loosening rate at 6 months after the operation was 0 in both groups,and the implant retention rate in the observation group was 97.06%,which was no significant difference to the control group(100%)(P>0.05);There were no significant differences in PIS,mPLI,MSBI and PD between the two groups(P>0.05);the preoperative and postoperative gingival crevicular fluid IL-2,IL-8 levels of the observation group were sharply higher than those of the control group,there was statistical significance(P<0.05),and the preoperative,postoperative results of each group are not significant(P>0.05).Conclusion:Oral implant technology has good recent curative effect on patients with chronic periodontitis dentition defect,it keeps PIS,mPLI,MSBI and PD excellent and will not increase the peri implant inflammation.
