术前辅助化学治疗在晚期卵巢上皮癌治疗中的意义

摘　　要：目的探讨晚期卵巢上皮癌的治疗方法,术前化学治疗的可行性及意义。方法回顾性分析Ⅲc至Ⅳ期的晚期卵巢上皮癌患者116例,分为2组,54例术前辅助化学治疗组及62例直接手术组。2组病例一般状况采用t检验,年龄、分期、病理类型和分化差异无统计学意义(P>0.05),观察2组病例的手术满意率(采用χ~2检验)及生存时间(生存分析采用Kaplan-meier法进行,生存率差异比较应用Log-rank法检验)。结果术前辅助化学治疗组理想减灭术(残余瘤<1 cm)和非理想减灭术分别为34/54(63%),20/54(37%)例,而直接手术组则分别为34/62(55%),28/62(45%)例,差异无统计学意义(P=0.779)。从分期、病理类型及分化3个方面进行比较,2组的1年~,2年~,3年~生存率及中位生存时间差异无统计学意义(P>0.05)。结论术前辅助化学治疗并不能明显改善预后,延长生存期。尽量先行肿瘤细胞减灭术,对于一般状况差,年龄太大者可行2~3周期的术前化学治疗。Objective To investigate the treatment of advanced epithelial ovarian cancer,the feasibility and significance of preoperative chemotherapy.Methods A retrospective analysis of 116 patients with advanced epithelial ovarian cancer stageⅢc-Ⅳwere divided into two groups,54 cases of preoperative adjuvant chemotherapy group and 62 cases in surgery group.The general condition of two groups using t test,age,staging,pathological type,differentiation(P>0.05)was no significant difference between the two groups were observed,surgery satisfaction rate(byχ2est)and survival time(survival analysis by Kaplan-meier method,the difference in survival compared using Log-rank test method).Results Preoperative adjuvant chemotherapy group ideal cytoreductive surgery(residual tumor less than 1 cm)and non ideal cytoreductive surgery were 34/54(63%),20/54(37%),and surgery group were 34/62(55%),28/62(45%),P=0.779,no statistical significance.From three aspects of staging,pathological type and differentiation ratio A group of two,1 year-group,2 year-group,3 year-group had no difference in survival rate and median survival time(P>0.05).Conclusion Preoperative adjuvant chemotherapy can significantly improve the prognosis and prolong the survival period.As far as possible first cytoreductive surgery,the general situation,preoperative chemotherapy is too large and feasible 2-3 cycle.
