细胞蜡块联合免疫组化技术在提高肺癌胸水细胞学阳性检出率及提供分型诊断方面的应用价值分析

摘　　要：目的观察细胞蜡块联合免疫组化技术在提高肺癌胸水细胞学阳性检出率及提供分型诊断方面的价值。方法对我院确诊肺癌患者的胸水标本58例,采用液基细胞学和细胞蜡块联合免疫组化两种制片方法,观察诊断的灵敏度和特异度,并对48例细胞蜡块诊断为阳性的病例进一步行免疫组化染色以分型,以活检病理诊断结果为标准,评价细胞蜡块结合免疫组化在分型诊断中的准确率。结果 58例肺癌患者胸水癌细胞检测中,液基细胞学灵敏度和特异度分别为81.25%、60.00%,明显低于细胞蜡块联合免疫组化染色法的95.83%、90.00%(P<0.05)。细胞蜡块联合免疫组化染色法检查分型总准确率为95.83%。结论细胞蜡块联合免疫组化技术能够明显提高肺癌胸水细胞的阳性检出率,为癌症分型诊断提供可靠依据。Objective To observe the value of cellular wax block combined with immunohistochemical technique in improving the positive rate of pleural effusion cytology and providing typing diagnosis of lung cancer.Methods Fifty-eight specimens of pleural effusion of patients with lung cancer confirmed by our hospital were treated with two methods of liquid based cytology and combined immunohistochemical staining of cell wax blocks.The sensitivity and specificity of diagnosis were observed.Forty-eight cases with positive cell wax were further stained by immunohistochemical staining and the results of biopsy were used as criteria to evaluate the accuracy of cell wax combined with immunohistochemical staining in typing diagnosis.Results In 58 cases of lung cancer,the sensitivity and specificity of liquid based cytology were 81.25%and 60.00%,respectively,which were significantly lower than those of cell block combined with immunohistochemical staining 95.83%and 90.00%(P<0.05).The total accuracy of cell wax block combined with immunohistochemical staining was 95.83%.Conclusion Cell wax block combined with immunohistochemical technique can significantly improve the positive detection rate of lung cancer pleural effusion cells,and provide a reliable basis for the diagnosis of cancer typing.
