血栓通注射液对急性脑梗死患者脑血流动力学及血清指标的影响

摘　　要：目的观察血栓通注射液对急性脑梗死(ACI)患者脑血流动力学及血清神经元特异性烯醇化酶(neuron-specific enolase,NSE)、胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)的影响。方法选取80例急性脑梗死患者,按入院顺序随机分为对照组(n=40)和观察组(n=40),对照组给予常规治疗,观察组在常规治疗的基础上加用血栓通注射液,共治疗2周。比较治疗前后两组NIHSS评分及Fugl-Meyer运动功能评分(FMA),以及脑血流动力学指标和血清NSE、IGF-1水平变化。结果治疗2周后,两组NIHSS评分较治疗前明显降低(P<0.05),且治疗后观察组NIHSS评分明显低于对照组(P<0.05);两组FMA评分均较治疗前明显提高(均P<0.01),且观察组评分明显高于对照组(P<0.05)。治疗后两组脑血流动力学指标均较治疗前明显升高(P<0.05),观察组治疗后血流动力学指标较对照组升高明显(P<0.05);两组血清NSE水平均较治疗前明显降低(P<0.05),而血清IGF-1水平较治疗前则明显升高(P<0.05);观察组治疗后血清NSE水平较对照组下降明显(P<0.05),而血清IGF-1水平较对照组则明显升高(P<0.05)。结论血栓通注射液能有效改善ACI患者神经功能及脑血流动力学,降低血清NSE水平,提高血清IGF-1水平,具有保护神经功能的作用。Objective To investigate the effects ofxueshuantong injection on cerebral haemodynamic,serum neuron-specific enolase(NSE)and insulin-like growth factor-1(IGF-1)in patients with acute cerebral infartion.Methods 80 patients with acute cerebral infarction were randomly divided into observation group(n=40)and control group(n=40)by order of admission.The patients in the control group were received anticoagulant drugs,thrombolytic drugs and so on.The patients in the observation group were given xueshuantong injection for 2 weeks.NIHSS score and FMA score were compared between two groups after 2w treatment.Cerebral haemodynamic,the levels of serum NSE and IGF-1 in two group were detected before and after 2w treatment.Results After 2w treatment,NIHSS score in two groups was lower than pretreatment(P<0.01 or 0.05),NIHSS score was significantly lower in observation group than that in control group.FMA score in both groups were significantly higher than before treatment(all P<0.01),and FMA score in observation group was significantly higher than that in control group(P<0.05).After 2w treatment,cerebral haemodynamic parameters in two groups was higher than those before treatment(P<0.01 or 0.05),cerebral haemodynamic parameters in observation group were significantly higher than those in control group(all P<0.05);After 2w treatment,the level of serum NSE in both groups was significantly decreased(P<0.01 or 0.05),the level of serum IGF-1 in both groups was significantly(P<0.01 or 0.05).The levels of serum NSE in observation group was significantly lower than that in control group(P<0.05),the level of serum IGF-1 in observation group was significantly higher than that in control group(P<0.05).Conclusion Xueshuantong injection can improve neurologic function and cerebral haemodynamic,reduce the level of NSE,increase the level of serum IGF-1 in acute cerebral infarction patients,and has the function of the neural function protection.
