HE4和CA125在高、低级别浆液性卵巢癌中表达差异的对照研究

摘　　要：目的:研究人附睾蛋白4(human epididymal secretory protein E4,HE4)和CA125在低级别浆液性癌(lowgrade serous carcinoma,LGSC)和高级别浆液性癌(high-grade serous carcinoma,HGSC)组织和血清中的差异。方法:以两级分级系统(two-tier grade system)将60例浆液性卵巢癌分为Ⅰ型LGSC和Ⅱ型HGSC,酶联免疫吸附(ELISA)双抗体夹心法检测患者血清中HE4和同位素法检测血清CA125的水平,免疫组化检测HE4、突变型P53蛋白在卵巢癌组织中的表达。分析卵巢癌患者血清中HE4、组织中HE4及血清CA125的表达水平在亚型卵巢浆液性囊腺癌的差异。结果:(1)LGSC和HGSC组织中HE4蛋白阳性率差异有统计学意义(P<0.01)。(2)HGSC的血清中HE4水平高于LGSC(P<0.01)。(3)FIGO(Ⅰ+Ⅱ期)中,LGSG占大多数75%;FIGO(Ⅰ+Ⅱ期)的LGSC和HGSC的组织HE4蛋白有表达差异,但血清HE4水平无差异(P>0.05)。血清CA125水平在LGSC和HGSC中无差异(P>0.05)。结论:(1)HGSC和LGSC组织和血清中的HE4蛋白表达有差异,提示HE4在鉴别低、高级别浆液性癌的潜在意义。(2)早期病例FIGO(Ⅰ+Ⅱ期)中,血清HE4在HGSC和LGSC中无统计学意义,同时HGSC早期病例比例小,故不能肯定血清HE4对HGSC的早期诊断的价值。(3)血清CA125水平在HGSC和LGSC无统计学意义,提示CA125无鉴别HGSC和LGSC的意义。Objective:To investigate the difference of human epididymal protein E4(HE4)and carbohydrate antigen 125(CA125)in low-grade serous carcinoma(LGSC)and high-grade serous carcinoma(HGSC)tissues and serum.Methods:Sixty cases of serous ovarian cancer were classified into typeⅠLGSC and typeⅡHGSC by two-tier grade system.Serum levels of HE4 and CA125 were measured by ELISA and isotope method respectively.The expression of HE4 and TP53 protein in ovarian cancer tissue were measured by immunohistochemistry.The expression of serum HE4,HE4 and serum CA125 in patients with ovarian cancer were analyzed in subtype ovarian serous cystadenocarcinoma.The differences in the expression level of HE4 in serum and in tissue and the expression level of CA125 in serum of patients with ovarian cancer in subtype ovarian serous cystadenocarcinoma were analyzed.Results:There was a significant difference in the positive rate of HE4 protein between LGSC and HGSC tissues(P<0.01).The level of HE4 in serum of HGSC was higher than that of LGSC(P<0.01).In Figo(stageⅠ+Ⅱ),LGSG accounted for the majority(75%).The expression of HE4 protein in LGSC and HGSC tissues was different from each other,but there was no difference in serum HE4 level(P>0.05).There was no difference in serum CA125 level between LGSC and HGSC(P>0.05).Conclusion:The difference of HE4 protein expression between tissue and serum of HGSC and LGSC suggests the potential significance of HE4 in the differential diagnosis of low and high grade serous carcinoma.In the early stage of Figo(stageⅠ+Ⅱ),there is no difference in serum HE4 between HGSC and LGSC,and the proportion of early HGSC cases was small.So the value of serum HE4 in early diagnosis of HGSCis not confirmed.No difference in serum CA125 levels between HGSC and LGSC insdicates that CA125 has no significance in differentiating HGSC from LGSC.
