球头硅胶管植入治疗慢性泪囊炎急性发作的临床疗效

摘　　要：目的:观察逆行泪道内植入球头硅胶管治疗慢性泪囊炎急性发作(AD)的临床疗效。方法:回顾性分析72例72眼慢性泪囊炎急性发作患者局部麻醉下逆行植入球头硅胶管,术后1周开始冲洗泪道,3个月拔管。观察其治疗效果及并发症情况。结果:61例61眼治疗后3~7天炎症迅速消退,合并瘘管者瘘管愈合,溢泪症状消除。随访观察6~12个月,无溢泪复发,泪囊区皮肤愈合良好,部分遗留色素沉着或轻度疤痕。治愈率84.72%。结论:逆行植入球头硅胶管治疗慢性泪囊炎急性发作,疗效好,操作简单,创伤小。由于此手术方式未改变泪道解剖结构,对慢性泪囊炎急性发作患者可作为首选治疗方法。Objective:To observe the clinical efficacy of retrograde spherical headed silicon tube implantation in the treatment of acute attack of chronic dacryocystitis.Methods:Seventy-two cases(72 eyes)of acute chronic dacryocystitis with retrograde implantation of bulbous silicone tube under local anesthesia were analyzed retrospectively.Lacrimal duct was douched one week after operation and the silicon tube was removed three months later.The treatment effect and complications were observed.Results:After being treated for three to seven days,the inflammation of 61 patients(61 eyes)rapidly subsidised;fistula was healed and the symptoms of epiphora were immediately resoluted.Follow-up for 6 to 12 months showed no recrudescence of the symptoms of epiphora.The skin of lacrimal sac district healed well,but part of them with skin pigmentation or mild scar.The cure rate was 84.72%.Conclusion:Retrograde spherical headed silicon tube implantation was an effective,simple,less invasive technique in the treatment of acute attack of chronic dacryocystitis.Because this operation does not change the anatomical structure of lacrimal duct,it can be used as the preferred method of treatment in patients with chronic dacryocystitis.
