摘 要:目的:探讨四维彩超在胎儿唇裂畸形产前诊断中的价值及效果,为临床诊断提供参考。方法:选取2015年5月-2017年5月本院经二维超声检查后疑似唇裂畸形胎儿20例,通过四维彩超进行二次诊断,对比诊断结果。结果:20例唇裂胎儿中,单纯唇裂5例,二维超声检查确诊2例(40.00%),四维彩超检查确诊5例(100%),比较差异有统计学意义(P<0.05);唇裂合并完全性腭裂14例,二维超声检查确诊7例(50.00%),四维彩超检查确诊12例(85.71%),比较差异有统计学意义(P<0.05);唇裂合并不完全性腭裂1例,二维超声与四维彩超均未诊出。15例单侧唇腭裂,二维超声误诊6例(40.00%),四维彩超误诊1例(6.67%),比较差异有统计学意义(P<0.05);5例非单侧唇腭裂,二维超声误诊4例(80.00%),四维彩超误诊0例,比较差异有统计学意义(P<0.05)。结论:四维彩超应用于产前诊断胎儿唇裂畸形具有良好的效果,二维超声对单纯唇裂和唇裂合并完全性腭裂的诊断率基本相似,而四维彩超对唇裂合并完全性腭裂的诊断准确率明显较高,且能准确判断唇裂为单侧还是非单侧,误诊率较低,值得在临床推广应用。Objective:To explore the value and effect of four-dimensional color Doppler ultrasound in prenatal diagnosis of fetal cleft lip deformity,to provide reference for clinical diagnosis.Method:20 cases of suspected cleft lip fetus after two-dimensional ultrasound examination from May 2015 to May 2017 in our hospital were selected,through four-dimensional color Doppler ultrasound for two times diagnosis,the diagnosis results were compared.Result:In 20 cases of cleft lip,5 cases of simple cleft lip,2 cases(40.00%)were diagnosed by two-dimensional ultrasound,5 cases(100%)were diagnosed by four-dimensional color Doppler ultrasound,the difference was statistically significant(P<0.05).14 cases of cleft lip with complete cleft palate,7(50.00%)cases were diagnosed by two-dimensional ultrasound,12 cases(85.71%)were diagnosed by four-dimensional color Doppler ultrasound,the difference was statistically significant(P<0.05).1 case of cleft lip with incomplete cleft palate were not diagnosed by two-dimensional ultrasound and four-dimensional color Doppler ultrasound.15 cases were unilateral cleft lip and palate,6 cases(40.00%)were misdiagnosed by two-dimensional ultrasound,1 case(6.67%)were misdiagnosed by four-dimensional color Doppler ultrasound,the difference was statistically significant(P<0.05).5 cases of non unilateral cleft lip and palate,4 cases(80.00%)were misdiagnosed by twodimensional ultrasound,0 case were misdiagnosed by four-dimensional color Doppler ultrasound,the difference was statistically significant(P<0.05).Conclusion:Four-dimensional color Doppler ultrasound has a good effect on prenatal diagnosis of fetal cleft lip deformity,compared with two-dimensional ultrasound,two-dimensional ultrasound has a similar diagnosis rate for cleft lip and cleft lip with complete cleft palate,while four-dimensional color Doppler ultrasound has higher accuracy in diagnosis of cleft lip with complete cleft palate,and can accurately determine the cleft lip is unilateral or non-unilateral,and the misdiagnosis rate is low,so it is
