色素胃镜与普通胃镜对食管早癌及癌前病变检出率的对比研究

摘　　要：目的:对比色素胃镜与普通胃镜对食管早癌及癌前病变检出率。方法:收治普通胃镜疑似早癌10例,疑似癌前病变26例;NBI胃镜检查发现的疑似早癌25例,疑似癌前病变的患者55例,退镜前NBI胃镜组均碘染色后进行靶向活检,病理确诊早癌及高级别内瘤变者行EMR或ESD术,最后根据病理结果进行统计分析。结果:普通胃镜早癌5例,NBI+碘染色胃镜早癌22例,检出率(50.0%,88.0%),差异有统计学意义(P<0.05),普通胃镜癌前病变16例,NBI+碘染色胃镜53例,检出率(64.0%,96.4%),差异有统计学意义(P<0.05)。结论:色素(NBI+碘染色)胃镜在食管早癌及癌前病变中的检出率与普通胃镜相比存在差异,可达到早发现、早诊断的目的。Objective:To compare the detection rate of early esophageal cancer and precancerous lesions between the pigmented gastroscope and the general gastroscope.Methods:10 cases of early carcinoma of the general gastroscopy and 26 precancerous lesions were suspected.25 cases of suspected early cancer and 55 cases of precancerous lesions were found by NBI gastroscopy.The target biopsy was performed in the NBI gastroscopy group before retrograde microscopy after iodine staining,EMR or ESD was performed by pathological diagnosis of early cancer and high grade neoplasia.Finally,the pathological results were analyzed according to the pathological results.Results:5 cases of early cancer were detected by ordinary gastroscope and 22 cases of early cancer with NBI+iodine staining gastroscopy were found,the detection rate was(50%,88%)(P<0.05).16 cases of precancerous lesions by gastroscopy,53 cases with NBI+iodine staining gastroscopy,the detection rate was(64%,96.4%)(P<0.05).Conclusion:The detection rate of pigmented(NBI+iodine staining)gastroscope in early esophageal cancer and precancerous lesions is different from that of conventional gastroscopy,so it can achieve the purpose of early detection and early diagnosis.
