摘　　要：目的探索自锁托槽矫治器用于牙颌畸形患者正畸中的临床价值。方法选取2015年3月11日~2016年3月11日期间我院收治的牙颌畸形患者100例,将其随机化分组,两组各50例,对照组和观察组分别采用传统金属托槽治疗和自锁托槽矫治器治疗,随后对比两组患者的矫治初期疼痛程度、托槽脱落情况、菌斑指数(PLI)、牙龈指数(GI)、龈沟出血指数(SBI)、谷草转氨酶(AST)含量。结果观察组患者的PLI(1.14±0.36)、GI(1.02±0.05)、SBI(1.52±0.12)、治疗后24 hAST含量(95.47±5.69)×10^(-6) U/L、治疗后48 hAST含量(202.98±5.37)×10^(-6) U/L、治疗后168 h AST含量(321.77±11.05)×10^(-6)U/L、3个月托槽脱落率(0)、6个月托槽脱落率(0)、12个月托槽脱落率(2.00%)、治疗后24 h疼痛评分(1.15±0.23)分、治疗后48 h疼痛评分(1.02±0.38)分、治疗后72 h疼痛评分(0.13±0.09)分均优于对照组(P<0.05)。结论对牙颌畸形患者实施自锁托槽矫治器治疗效果显著。Objective To explore the clinical value of self-ligating appliance for orthodontics in the patients with dento-mandibular deformity.Methods 100 patients with dento-mandibular deformity who were admitted to our hospital from March 11,2015 to March 11,2016 were selected.The patients were divided into two groups according to random method,with 50 patients in each group.The control group and the observation group were given traditional metal brackets and self-ligating appliance respectively for treatment.Afterwards,the pain level,bracket detachment,plaque index(PLI),gingival index(GI),sulcus bleeding index(SBI)and aspartate aminotransferase(AST)were compared between the two groups at the initial stage of correction.Results In the patients in the observation group,the PLI was(1.14±0.36),GI was(1.02±0.05),SBI was(1.52±0.12),24 hours after treatment AST content was(95.47±5.69)×10-6 U/L,48 hours after treatment AST content was(202.98±5.37)×10-6 U/L,and 168 hours after treatment AST content was(321.77±11.05)×10-6 U/L,the 3-month brackets detachment rate was 0,6-month brackets detachment rate was 0,and 12-month brackets detachment rate was 2.00%,the pain scores at 24 hours after treatment was(1.15±0.23),the pain score at 48 hours after treatment was(1.02±0.38),and the pain scores at 72 hours after treatment was(0.13±0.09),which were significantly better than those in the control group(P<0.05).Conclusion The self-ligating appliance for orthodontics in the patients with dento-mandibular deformity has a significant curative effect.
