HER2在胃癌原发灶和淋巴结转移灶中表达的对比研究

摘　　要：目的:对比胃癌原发灶和淋巴结转移灶中人表皮生长因子受体2(human epidermal growth factor receptor2,HER2)蛋白表达、基因扩增情况。方法 :收集南通大学附属南通第三医院2014年1月—2017年8月46例胃癌伴有淋巴结转移的手术病例,将所有原发灶和淋巴结转移灶标本全部取材,行HER2蛋白免疫组织化学(immunohistochemistry,IHC)和荧光原位杂交(fluorescence in situ hybridization,FISH)检测。结果 :IHC显示:胃癌原发灶HER2蛋白过表达率为23.9%,且与肿瘤分化程度、Lauren分型相关(P<0.05);FISH检测结果显示:原发灶HER2表达0/1+的病例均无扩增,表达2+的病例扩增率为16.7%,表达3+的病例扩增率为100.0%。淋巴结转移灶HER2蛋白的过表达率为21.7%,与淋巴结转移数量之间无明显相关性(P>0.05),原发灶和转移灶HER2蛋白的过表达率差异无统计学意义(P>0.05);转移灶HER2表达0/1+的病例均无扩增;表达2+的病例扩增率为16.7%;表达3+的病例扩增率为100%。46例原发灶和对应淋巴结转移灶中,HER2蛋白表达一致率为93.4%,基因扩增一致率为97.8%。结论:通过比较胃癌原发灶和转移灶中HER2的表达,发现原发灶和转移灶的表达具有很高的一致性。Objective:To compare the expression of human epidermal growth factor receptor 2(HER2)protein and gene amplification in the primary and lymph node metastases of gastric cancer.Methods:46 cases of gastric cancer with lymph node metastasis from January 2014 to August 2017 in Affiliated Nantong Third Hospital of Nantong University were collected.All specimens of primary and lymph node metastases were collected.HER2 protein immunohistochemistry(IHC)and fluorescence in situ hybridization(FISH)were performed.Results:IHC showed the overexpression of HER2 protein in primary gastric cancer was 23.9%,and was associated with the degree of tumor differentiation and Lauren typing(P<0.05).The results of FISH detection showed that no amplification was found in the cases of HER2 expressing 0/1+in primary foci,and the amplification rate of 2+was 16.7%,and the amplification rate of 3+was 100.0%.The overexpression of HER2 protein in lymph node metastases was 21.7%,and there was no significant correlation with the number of lymph node metastases.There was no significant difference between the primary and metastatic foci(P>0.05).No amplification was found in the cases of HER2 expressing 0/1+in the metastatic foci,and the amplification rate of 2+was 16.7%,and the amplification rate of 3+was 100.0%.In 46 cases of primary and corresponding lymph node metastases,the consistent rate of HER2 protein expression was 93.4%,and the consistent rate of gene amplification was 97.8%.Conclusion:By comparing the expression of HER2 in the primary and metastatic foci of gastric cancer,it was found that the expression of the primary and metastatic foci was very consistent.
