不同医联体管理模式下医师能力提升的探究

摘　　要：目的观察紧密型和松散型医联体管理模式下医师能力提升差异。方法比较两种医联体模式下三基知识考试成绩以及不同职称医师的考试成绩。结果理论成绩、实践成绩、总成绩3项得分中紧密型组成绩明显优于松散型组(P<0.05);比较不同职称医师三基知识水平考试成绩,除了理论成绩的提升无组间差异外(P>0.05),实践成绩和总成绩均有组间差异,紧密型医联体在住院医师实践能力提升明显优于副高及主治组别。结论建立"核心医院科室-基层成员医院"的紧密型医联体,以联合病房(病区)为载体,促进人力资源有效双向流动,搭建多形式、多维度医疗支援平台,是扎实提高医联体内涵质量,做到"分级诊疗不减质,双向转诊增实效"的有效途径。Objective To compare the difference of physician ability improvement between the tight and loose medical consortium management modes.Methods The results of the three basic knowledge tests and the examination results among different professional titles were compared under the two medical consortium management modes.Results The theoretical score,practice score and total score of the tight mode group were significantly better than those of the loose mode group(P<0.05).In comparison of three basic knowledge tests scores among different professional titles,no difference was found between the groups on the theoretical score(P>0.05),and the practice scores and total scores had difference between groups,which showed that the tight medical consortium management mode had better improvement on residency’s practice ability than that of the associated and the attending groups.Conclusion The establishment of the“core hospital departments and primary members hospitals"tight compact medical consortium,with joint ward as an effective carrier to promote effective two-way flow of human resources and build a multi-form and multi-dimension medical support platform,is an effective way to promote the connotation quality of medical consortium,and to ensure the“high quality of hierarchical diagnosis and treatment,and effectiveness of two-way referral”.
