^(18)F-FDG摄取程度、Ki-67表达与DLBCL分期之间的相关性讨论

摘　　要：目的探讨弥漫大B细胞淋巴瘤^(18)F标记的氟代脱氧葡萄糖(^(18)F-FDG)摄取程度、核抗原Ki-67表达与临床分期之间的关系。方法回顾性分析我院70例弥漫大B细胞淋巴瘤患者的病历资料,所有患者均经病理及免疫组化结果确诊,治疗前均常规行^(18)F-FDG PET/CT扫描。结果弥漫大B细胞淋巴瘤患者病灶^(18)F-FDG平均摄取值(SUVave)与Ki-67指数呈显著正相关,相关系数r为0.843,P<0.001;Ki-67指数与淋巴瘤分期显著相关(P<0.001),且Ki-67指数越高,临床分期越趋向晚期;^(18)F-FDG平均摄取值与淋巴瘤分期无相关性。弥漫大B细胞淋巴瘤患者Ki-67表达水平与肿瘤分期、最大肿瘤直径及血LDH水平有相关性(P<0.05)。结论 Ki-67与PET/CT在反映弥漫大B细胞淋巴瘤患者细胞增殖情况及病灶的侵袭程度等方面有着紧密的联系。Objective To discuss the correlation and clinical signification arnorg 18 F-FDG uptake level,the expression of nuclear antigen Ki-67 and staging of DLBCL.Methods We had retrospectively analyzed the imaging materials and results of 70 DLBCL patients,all the patients were diagnosed by the pathology and immunohistochemistry.Before the treatment,all the patients had been examined by PET/CT routinely.Results The 18 F-FDG average standard uptake value of DLBCL patients is positively correlated with Ki-67,the correlation coefficient r is 0.843(P<0.001).There is a correlation between Ki-67 and staging(P<0.001),the higher the index is,the more advanced the staging is.There is no correlation between 18 F-FDG average standard uptake value and staging.There is a statistical difference between Ki-67 expression level and tumor staging,largest tumor diameter,blood LDH level on DLBCL patients(P<0.05),and there is no statistical difference with other clinical factors.Conclusion There is a close relationship between Ki-67 and PET/CT in reflecting cell proliferation and invasion of the lesions on DLBCL patients.
