MCV、MCH联合HGB电泳对地中海贫血筛查的应用价值探讨

摘　　要：目的:研究探讨平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)联合血红蛋白(HGB)电泳对地中海贫血筛查的应用价值,为临床有效诊断地中海贫血提供最佳的方案。方法:选取接受检查的地中海贫血患者80例为观察组,另取同期于我院接受检查的非地中海贫血患者80例记为对照组。分别对两组患者进行MCV、MCH、HGB电泳检查,对比两组患者各项检查的阳性率,并对三种检查方式以及三项联合检查的灵敏度、特异度、阳性预测值以及阴性预测值情况进行对比分析。结果:观察组患者MCV、MCH、HGB电泳单项检查的阳性率分别为73例(91.25%)、75例(93.75%)、70例(87.50%),均显著高于对照组的19例(23.75%)、20例(25.00%)、18例(22.50%),差异均有统计学意义(P<0.05)。观察组患者MCV、MCH、HGB电泳三项联合检查的阳性率为95.00%(76/80),显著高于对照组的3.75%(3/80),差异均有统计学意义(P<0.05)。三项联合检测的灵敏度、特异度、阳性预测值以及阴性预测值分别为95.00%、96.5%、96.20%、95.06%,均显著高于MCV、MCH、HGB电泳单项检查,差异均有统计学意义(P<0.05)。结论:对于地中海贫血患者而言,MCV、MCH、HGB电泳三项联合检查可有效提高疾病诊断的灵敏度以及特异度,具有极其重要的临床应用价值,值得推广应用。Objective To study the average red blood cell volume(MCV),average red blood cell hemoglobin content(MCH),hemoglobin(HGB)electrophoresis of thalassaemia screening application value,provide the best solution for clinical effective diagnosis of thalassemia.Method Selected for inspection of the Mediterranean anemia patients 80 cases as the observation group,the other in the same period in our hospital for inspection of the Mediterranean anemia patients 80cases as control group.Respectively on two groups of patients with the MCV,MCH,HGB electrophoresis examination,compared two groups of patients with positivepredictive value of the check,and check for three kinds of ways and three joint inspection sensitivity,specific,positive predictive value and negative predictive value were analyzed.Results The observation group of patients the MCV,MCH,electrophoresis HGB single check the positive rate was 91.25%(73/80),93.75%(75/80),87.50%(70/80),were significantly higher than that of control group of 23.75%(19/80),25.00%(20/80),22.50%(18/80),the difference had statistical significance(P<0.05).Observation group of patients the MCV,MCH,electrophoresis HGB three joint inspection of the positive rate was 95.00%(76/80),significantly higher than thatof control group 3.75%(3/80),the difference had statistical significance(P<0.05).Three joint detection sensitivity,specific degree,positive predictivevalue and negative predictive value were 95.00%,96.50%,96.20%,96.50%were significantly higher than the MCV,MCH,electrophoresis HGB single inspection,were statistically significant differences(all P<0.05).Conclusion For patients with Mediterranean anemia,MCV,MCH,electrophoresis HGB three joint inspection can effectively improve thesensitivity of diagnosis and specific degrees,has extremely important clinicalapplication value,worthy of popularization and application.
