摘 要:子宫内膜癌(EC)的危险级别取决于癌灶的生长浸润范围、组织学类型和分化程度,准确判断以上因素对选择治疗方案和判断病人预后十分重要。MRI的软组织分辨力较高,可以清晰地显示EC与子宫肌层、宫颈及宫旁邻近组织器官的关系,在判断癌灶的生长浸润范围方面具有很高的准确性。MR扩散加权成像能在一定程度上预测EC癌灶的组织学类型和分化程度,因此MRI已成为术前评估EC危险级别最常用的检查方法。相比宫腔镜直视下内膜活检,MRI可以观察子宫内膜的整体情况,更易发现局部体积很小的癌灶,可以弥补内膜活检因取材的局限性而造成的部分假阴性。就MRI多序列对EC的诊断、评估其生长浸润范围以及预测组织学类型和分化程度的研究进展予以综述。The severity of endometrial cancer(EC)depends on the infiltration extent,histological type and differentiation degree of the tumor,and it is very important to accurately evaluate the above factors to design the treatment plan and estimate the prognosis.With high resolution,magnetic resonance imaging(MRI)can clearly shown the anatomical relationship of EC with myometrium,cervix and adjacent tissues of the uterus.Therefore it can accurately evaluate infiltration extent of EC.Meanwhile,diffusion-weighted imaging can be used to estimate the histological type and cellular differentiation degree of tumor to some extent.So,MRI has become the most common method to evaluate the severity of EC.Compared with biopsy,MRI can be used to observe the overall condition of the endometrium,and it is more likely to detect a small foci lesion,which is usually false negative with biopsy.In this paper,we reviewed the progress of multiple sequence MRI in the detection EC and evaluation the infiltration extent,histological type and cellular differentiation degree of the EC.
