康复训练在四肢肌间血管畸形手术中的作用评价

摘　　要：目的观察手术治疗四肢肌间血管畸形后早期康复训练与否的肢体功能差别,探讨康复训练在手术治疗肌间血管畸形中的作用。方法自2015年10月~2016年10月临沂市经济技术开发区人民医院和临沂市肿瘤医院共收治四肢肌间血管畸形患者147例,均接受手术治疗。按随机对照的原则分为观察组和对照组,其中观察组74例,术后系统康复训练,包括术后早期理疗、被动活动,拆线后的肌力负重锻炼及必要的器械训练;对照组共73例,术后无康复训练干预。比较两组临床疗效。结果两组负压引流出血量无统计学差异(P>0.05)。术后随访1~2年,两组患者临床疗效无统计学差异。两组患者术前关节功能评价无统计学差异(P>0.05);患者术前和术后关节功能评价对比,观察组和对照组均有统计学差异(P均<0.05);两组患者术后关节功能评价有统计学差异(P<0.05)。结论手术切除是纠正血管畸形合并关节功能障碍的有效手段,早期康复训练并不增加术后出血风险,可进一步帮助改善术后关节功能。四肢血管畸形手术后应尽早进行系统的康复训练。Objective To observe the differences of function in extremities surgeries presented with vascular malformations with or without rehabilitation training,and to nvestigate its effect in theses surgeries.Methods Operations were performed in 147 cases of extremities vascular malformation,which were randomly classified into 2 groups that were called observation group and control group.There were 74 cases in observation group received prophase rehabilitation training,while 73 without these.The clinical effects of the two groups were compared.Results There were no differences in blood volume of negative pressure drainage between the two groups(P>0.05).There were no differences in both therapeutic effect and joint function between the two groups(P<0.05)before operation measures.After1 years to 2 follow-ups,there were differences in joint function between the two groups after operation measures(P<0.05),and there were also differences in joint function before and after operation measures in both two groups(P<0.05).Conclusion Surgical resection plays an important part in in the treatment of vascular malformation with joint dysfunction,early rehabilitation training does not increase the risk of postoperative bleeding,but can obviously improve joint function after operations of vascular malformations.Early rehabilitation training should be recomended after surgeries in extremities.
