摘  要:目的观察Onlay植骨技术在口腔种植中的应用效果。方法选取2015年9月至2017年10月郑州大学第一附属医院种植科收治的5例牙齿缺损患者作为研究对象,通过口腔专科检查和口腔颌面锥形束CT(CBCT)检查进行术前评估,制定合理的Onlay自体骨移植和种植手术方案,恢复缺牙区的骨量和缺失牙,手术后6个月通过CBCT检查检测牙槽骨骨密度,并统计种植牙存活情况。结果术后6个月,5例患者缺牙区牙槽嵴水平骨量提升的中位数为4.2 mm,垂直骨量提升的中位数为0.8 mm,患者口腔软组织、牙龈乳头、牙龈颜色均基本恢复正常,供骨区无神经症状、继发性感染及其他不适,种植牙存活率为100%。结论 Onlay自体骨植骨术是能够解决中度甚至重度骨萎缩或骨缺损的有效临床途径,能够使牙槽骨水平向及垂直向骨量均得到良好改善。Objective To observe the application effect of Onlay bone grafting technique in oral implants.Methods From September of 2015 to October of 2017,5 patients with dental defect enrolled in the Department of Impantology of the First Affiliated Hospital of Zhengzhou University were selected as research subjects.Based on preoperative assessment of dental examination by stomatological specialty and CBCT examination,a reasonable Onlay autogenous bone graft and implant operation scheme was established to restore the bone mass and missing teeth in the odontic area.The bone mineral density of the alveolar bone was detected by CBCT examination 6 months after operation,and the survival of the dental implant was counted.Results Six months after surgery,median horizontal bone mass of edentulous alveolar ridge was increased by 4.2 mm,and median vertical bone mass was increased by 0.8 mm in 5 patients.Oral soft tissue and gingival papilla,gingival color were returned to normal,and there was no neurological symptom,secondary infection and other discomfort in the donor site.The survival rate of dental implants was 100%.Conclusion Onlay autogenous bone graft is an effective way to solve moderate or even severe bone atrophy or bone defect.It can improve the horizontal and vertical bone mass of alveolar bone.
