胰腺实性假乳头状瘤MSCT表现特点分析

摘　　要：目的分析胰腺实性假乳头状瘤(SPTP)的多层计算机断层摄影术(MSCT)的表现特点,提高其诊断水平。方法回顾性分析36例经手术病理证实的SPTP患者的临床资料,对比影像学及病理学表现。结果 36例患者的平均年龄为26.6岁,其中女32例,男4例,胰头部15例,胰腺尾部13例,胰颈部4例,胰腺体部4例。CT检查:36例均见肿块,囊实性29例(80.6%),实性为主者5例(13.9%),囊性为主者2例(5.6%);合并点或片状钙化11例,合并出血9例。增强后33例呈轻中度强化,3例重度不均匀强化;伴假包膜29例,伴体尾部萎缩5例,伴主胰管轻度扩张3例。结论 SPTP的MSCT表现具有一定的特点,年轻女性、具有假包膜的囊实性肿块,且轻中度、持续强化时,需考虑SPTP的可能。Objective To analyze the features of multisice computed tomography of solid-pseudopapillary tumor of pancreas and to improve its diagnosis.Methods The clinical data of thirty six patients with solid-pseudopapillary tumor of pancreas confirmed by surgery and pathology were retrospectively analyzed.The imaging characteristics and pathology were compared.Results Within thirty six cases,the average age was 26.6 years old,and thirty two cases were female,four cases were male.The tumor of fifteen cases localized at the head of pancreas,thirteen cases at tail,four cases at neck and four cases at body.On plain CT images:thirty-six cases were mass lesion,and twenty nine cases appeared solid-cystic mass(80.6%),five cases solid mass mainly(13.9%),and two cases cystic mass mainly(5.6%).Eleven cases were accompanied by punctate or flaky calcification,nine cases by hemorrhage.There were thirty three cases of mild to moderate enhancement,three cases of severe inhomogeneous enhancement.There were twenty nine cases of pseudocapsule,five cases of pancreatic body-tail atrophy,and three cases with mild dilatation of main pancreatic duct.Conclusion There are some characteristics of multisice computed tomography findings of solid-pseudopapillary tumor of pancreas,especially young women,solid-cystic tumor with pseudocapsule,and mild to moderate and continuous enhancement,the need to consider the possibility of solid-pseudopapillary tumor of pancreas.
