摘 要:目的探讨溃疡性结肠炎患者肠道菌群的变化特征。方法诊断为溃疡性结肠炎患者50例为溃疡性结肠炎组,同期筛选出50例身体健康者为健康组,对2组受试者的新鲜粪便进行菌群分析,同时检查2组受试者血清内毒素、白介素-10(IL-10)、白介素-4(IL-4)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平并分析上述炎性因子与菌群变化的相关性。结果溃疡性结肠炎组双歧杆菌属、乳酸杆菌属、消化球菌属及真杆菌属数量均低于健康组,而肠球菌属、小梭菌及肠杆菌数量溃疡性结肠炎组高于健康组,差异有统计学意义(P<0.05),2组酵母菌属数量比较差异无统计学意义(P>0.05);溃疡性结肠炎组双歧杆菌属、乳酸杆菌属、消化球菌属及真杆菌属阳性检出率均低于健康组,而肠球菌属、小梭菌及肠杆菌阳性检出率溃疡性结肠炎组高于健康组,差异均有统计学意义(P<0.05),酵母菌属阳性检出率两组间比较差异无统计学意义(P>0.05);溃疡性结肠炎组血清IL-10、IL-4水平低于健康组,而IL-6、TNF-α、内毒素水平溃疡性结肠炎组高于健康组,差异有统计学意义(P<0.05);Pearson相关性分析法结果显示,内毒素水平、TNF-α与双歧杆菌数量呈负相关(r值分别为-0.312、-0.265,P<0.05),与肠杆菌呈正相关(r分别为=0.341、0.329,P<0.05);IL-6与乳酸杆菌、真杆菌呈负相关(r值分别为了0.354、-0.321,P<0.05),与肠球菌呈正相关(r=0.283,P<0.05);IL-4与小梭菌呈负相关(r=-0.317,P<0.05),与双歧杆菌呈正相关(r=0.275,P<0.05);IL-10与消化球菌呈正相关(r=0.350,P<0.05)。结论溃疡性结肠炎患者的肠道菌群失衡,体内促炎因子分泌增多,抑炎因子水平降低。Objective To investigste the variation characteristics of intestinal flora in patients with ulcerative colitis.Methods A total of 50 patients with ulcerative colitis who were treated in our hospitals from June 2014 to June 2016 were enrolled as ulcerative colitis group,at the same time,50 healthy volunteers were enrolled as control group.The flora of fresh faeces was analyzed in both groups,moreover,nd the serum levels of inflammatory factors including of endotoxin,IL-10,IL-4,IL-6,TNF-αwere detected to analyze the correlation between the inflammatory factors and the variation of flora.Results The numbers of bifidobacterium,lactobacillus,peptococcus and bacillus genus in ulcerative colitis group were significantly lower than those in control group,however,the numbers of enterococcus,small clostridium and enterobacter in ulcerative colitis group were significantly higher than those in control group(P<0.05).There were no significant differences in the numbers of blastocystis between two groups(P<0.05).The positive detection rates of bifidobacterium,lactobacillus,digestive aureus and bacillus genus in ulcerative colitis group were significantly lower than those in control group,while the positive detection rates of enterococcus,small clostridium and enterobacter in ulcerative colitis group were significantly higher than those in control group(P<0.05).There was no significant difference in blastocystis positive detection rate between two groups(P<0.05).The serum levels of IL-10,IL-4 in ulcerative colitis group were significantly lower than those in control group(P<0.05).The serum levels of IL-6,TNF-α,endotoxin in ulcerative colitis group were significantly higher than those in control group(P<0.05).Pearson correlation analysis results showed that the levels of TNF-αand endotoxin were negatively correlated with those of bifidobacterium(r 1=-0.312,r 2=-0.265,P<0.05),however,which were positively correlated with those of enterobacterium(r 1=0.341,r 2=-0.329,P<0.05).The levels of IL-6 were negatively correlated with th
