摘　　要：目的探讨经颅多普勒超声(transcranialdoppler,TCD)在健康体检中对无症状脑血管狭窄患者的应用价值。方法选取2015年10月—2017年10月在德阳市人民医院健康体检中心通过TCD检查的18 032名体检者,对其TCD结果进行分析。结果 18 000例符合标准受检者,共检出颅内动脉狭窄的113例,狭窄发生率为0.6%,其中男性81例,女性32例,男性狭窄高于女性(χ~2=42.496,P<0.05));在狭窄部位上,主要为中动脉(74.3%)、前动脉(12.4%);在年龄分布上,颅内血管狭窄的发生率随着年龄的增长而增加;为危险因素上,提示高血压、糖尿病、血脂代谢异常与颅内动脉粥样硬化型狭窄相关。结论经颅多普勒超声检测能灵敏反应颅内大血管的血流动力学改变,可早期发现无症状的颅内动脉狭窄,从而及时干预,在很大程度上降低脑卒中的发病率、致残率。Objective To study the application value of transcranial Doppler sonography TCD in screening the intracranial artery stenosis in the healthy physical examination.Methods 18 032 cases of patients with TCD examination in our hospital from October 2015 to October 2017 were selected and the TCD results were analyzed.Results 18 000 cases met the standards,113 cases were tested with intracranial artery stenosis,and the stenosis incidence rate was 0.6%,including 81 males and 32 females,and the stenosis of males was higher than that of females(χ2=42.496,P<0.05),in terms of stenosis site,the distributing arteries was in the first place(74.3%)and prerolandic artery Rolando was in the second place(12.4%),in terms of age,with the increase of the age,the incidence rate of intracranial vascular stenosis increased,and the risk factors showed that the hypertension,diabetes and blood liquid metabolic disturbance were correlated with intracranial atherosclerosis.Conclusion The transcranial Doppler sonography can sensitively reflect the changes of hemodynamics of intracranial vessel,and it can discover the intracranial artery stenosis without symptoms,thus greatly reducing the morbidity and disability of stroke.
