分根术联合核桩联冠修复治疗下颌第一磨牙根分叉病变的疗效分析

摘　　要：目的研究下颌第一磨牙跟分叉病变患者采用分根术联合核桩联冠修复治疗的临床效果。方法临床纳入84例该院2014年8月—2016年8月期间收治的下颌第一磨牙分叉病变的患者作为研究对象,按随机数字表法分为两组各42例。其中42例患者行分根术联合核桩单冠修复治疗作为单冠组,另42例患者行分根术联合核桩联冠修复治疗作为联冠组。观察两组治疗效果及并发症情况。结果联冠组患者治疗成功率高达95.24%,明显高于单冠组的80.95%(χ2=9.735,P<0.05)。两组患者术后并发症包括牙龈炎、牙周炎以及牙结石,联冠组发生率仅为7.14%,明显低于单冠组的19.05%(χ2=6.232,P<0.05)。结论下颌第一磨牙根分叉病变Ⅱ~Ⅲ度的患者采用分根术联合核桩联冠修复治疗效果显著,能明显修复下颌第一磨牙根分叉病变,术后并发症少,达到良好效果,值得临床应用及推广。Objective To research the curative effect of root separating and core-post crown in the treatment of mandibular first molar with root furcation involvement.Methods 84 cases of patients with first molar with root furcation involvement admitted and treated in our hospital from August 2014 to August 2016 were selected as the research objects,and randomly divided into two groups with 42 cases in each,the single crown group were treated with root separating and single-post crown,while the combined crown group were treated with root separating and core-post crown,and the treatment effect and complications were compared between the two groups.Results The treatment success rate in the combined crown group was obviously higher than that in the single crown group,(95.24%vs 80.95%),(χ2=9.735,P<0.05),and the postoperative complications of the two groups included gingivitis,periodontitis and dental calculus,and the incidence rate of combined crown group was obviously lower than that in the single crown group,(7.14%vs 19.05%),(χ2=6.232,P<0.05).Conclusion The effect of root separating and core-post crown in the treatment of mandibular first molar with root furcation involvement ofⅡ~Ⅲis obvious,which can obviously repair the mandibular first molar with root furcation involvement,with few complications and good effect,and it is worth clinical application and promotion.
