耳内窥镜在治疗耳胆脂瘤的临床应用

摘　　要：目的研究耳内窥镜在治疗外耳道胆脂瘤的临床应用价值。方法选取2013年2月—2016年3月于该院就诊的共63例胆脂瘤患者作为研究对象,按随机数字表法将所有患者分为对照组(n=29例)和研究组(n=34例)。其中对照组患者采用乳突根治术,研究组患者采用耳窥镜治疗,对比两组患者的听力提升情况、气骨导差改善情况、临床疗效及并发症发生情况。结果两组患者听力提升情况对比中,研究组正常率明显高于对照组,大腔、中腔、小腔等对比也差异有统计学意义(P<0.05)。两组患者气骨导差改善率对比,研究组改善率为100.00%,明显高于对照组,且差异有统计学意义(P<0.05)。对照组患者总有效率为75.86%;研究组患者总有效率为97.06%,两组患者总有效率对比差异有统计学意义(P<0.05)。对照组患者并发症发生率为41.38%;研究组患者并发症发生率为11.76%,两组患者并发症发生率对比差异有统计学意义(P<0.05)。结论耳内镜下进行外耳道胆脂瘤摘除具有简便易行、成功率高、操作准确和视野宽广等优点,降低术后并发症发生率,增加了手术的安全性,值得临床推广。Objective To research the clinical application value of otoendoscope in treatment of ear cholesteatoma.Methods 63 cases of cholesteatoma patients diagnosed in our hospital from February 2013 to March 2016 were selected as the research objects and randomly divided into two groups,the control group with 29 cases were treated with radical mastoidectomy,while the research group with 34 cases were treated with oto speculum,and the hearing improvement degree,improvement situation of air bone gap,clinical curative effect and occurrence of complications of the two groups were compared between the two groups.Results The normal rate in the research group was obviously higher than that in the control group,and the differences in the large cavity,medium cavity and small cavity between the two groups were statistically significant(P<0.05),the improvement rate of air bone gap in the research group was 100.00%,which was obviously higher than that in the control group,and the difference was statistically significant(P<0.05),and the difference in the total effective rate between the control group and the research group was statistically significant,(75.86%vs 97.06%)(P<0.05),and the difference in the incidence rate of complications between the control group and the research group was obvious(41.38%vs 11.76%)(P<0.05).Conclusion The endotoscope in removing the ear Cholesteatoma is simple and convenient with high successful rate,accurate operation and broad vision,which can reduce the incidence rate of postoperative complications and increase the safety of operation,and it is worth clinical promotion.
