胎儿脐带在后巩膜加固术中的临床应用

摘　　要：目的:观察胎儿脐带在后巩膜加固术中的应用效果。方法:选取54例(108只眼)高度近视患者作为观察对象,采用随机数字表法将其分为对照组和研究组各27例。对照组给予LASEK术,研究组给予胎儿脐带后巩膜加固术联合LASEK术,对比两组裸眼视力(UCVA)、最佳矫正视力(BCVA)、视网膜动脉前期的充盈时间、安全性指数、有效性指数,记录两组暗光下5 mm瞳孔直径下波前像差,包括垂直彗差(C7)、水平彗差(C8)和球差(C12)。结果:术后1、3个月,研究组UCVA、BCVA、C7、C8、C12水平均明显高于对照组(P<0.05);术后12个月研究组有效性指数和安全性指数均明显大于对照组(P<0.05),视网膜动脉前期的充盈时间明显短于对照组(P<0.05)。结论:胎儿脐带后巩膜加固术治疗高度近视的效果显著,患者可获得理想的视觉质量,是临床矫治高度近视的较佳方法。Objective:To observe application effects of posterior scleral reinforcement with fetal umbilical cord for high myopi-a.Methods:54 cases(108 eyes)with high myopia were selected and divided into control group(n=27)and study group(n=27)according to the random number table method.The control group(54 eyes)underwent LASEK surgery,while the study group(54 eyes)received posterior scleral reinforcement with fetal umbilical cord combined with LASEK.The postoperative uncorrected visual a-cuities(UCVA),best corrected visual acuities(BCVA),pre-retinal artery filling time,safety indexes,effectiveness indexes of the two groups were compared.Further,the 5min dark light pupil wavefront aberrations were recorded,including horizontal primary coma(C8),vertical primary coma(C7)and spherical aberration(C12).Results:Compared with the control group,the UCVA,BCVA,C7,C8 and C12 of the study group were significantly higher 1 and 3 months after the surgery(P<0.05).12 months after the surgery,the safety index and effectiveness index were obviously higher in the study group than the control group(P<0.05),and the pre-retinal artery filling time was significantly shorter than that of the control group(P<0.05).Conclusions:Posterior scleral reinforcement with fetal umbilical cord has significant effects in the treatment of high myopia,and can improve the patients'visual quality.Therefore,it is an ideal method for clinical correction of high myopia.
