龈上洁治术与牙周非手术治疗对单纯中重度牙周炎患者肱踝脉搏波传导速度、踝臂指数和sICAM-1的影响比较

摘　　要：目的比较龈上洁治术与牙周非手术治疗对单纯中重度牙周炎患者肱踝脉搏波传导速度、踝臂指数和血清可溶性细胞间黏附分子(sICAM-1)的影响。方法前瞻性选取106例单纯中重度牙周炎患者,随机分为常规组行龈上洁治术及研究组行牙周非手术治疗,两组各53例。比较两组患者治疗前后牙周指标[平均临床附着丧失(CAL)、探诊深度(PD)及探诊出血指数(BOP)]、肱踝脉搏波传导速度、踝臂指数和sICAM-1的变化情况。结果常规组患者治疗后1个月CAL、PD及BOP水平与治疗前比较均无显著差异(P>0.05),而研究组患者治疗后CAL、PD及BOP水平均优于治疗前(P均<0.05),且均优于常规组(P<0.05)。治疗1个月后,常规组肱踝脉搏波传导速度、踝臂指数和sICAM-1与治疗前比较均无显著差异(P>0.05),而研究组肱踝脉搏波传导速度与sICAM-1均低于治疗前(P<0.05),踝臂指数稍高于治疗前,但差异无显著性(P>0.05);治疗1个月后,研究组踝脉搏波传导速度与sICAM-1均明显较常规组低(P<0.05),而踝臂指数比较并无显著差异(P>0.05)。结论相比龈上洁治术,牙周非手术治疗更能够降低单纯中重度牙周炎患者肱踝脉搏波传导速度,降低sICAM-1的水平,进而有助于减轻发生心血管疾病的风险。Objective To compare the effects of supragingival scaling and non-surgical periodontal therapy on brachial-ankle pulse wave velocity,ankle brachial index and sICAM-1 in patients with moderate to severe periodontitis.Methods 106 cases of moderate to severe periodontitis were selected and divided into the conventional group received supragingival scaling and the study group underwent non-surgical periodontal therapy,with 53 cases in each group.The changes of periodontal parameters[mean clinical attachment loss(CAL),probing depth(PD),and probing bleeding index(BOP)],brachial-ankle pulse wave velocity,ankle brachial index and sICAM-1 were compared between the two groups before and after treatment.Results 1 months after the treatment,the levels of CAL,PD and BOP in the conventional group weren't significant difference with those before treatment(P>0.05).Those levels in the study group after treatment were better than those before treatment(P<0.05),and they were better than in the conventional group(P<0.05).After 1 months of treatment,the levels of brachial-ankle pulse wave velocity,ankle brachial index and sICAM-1 in the conventional group showed no significant difference with those before treatment(P>0.05).The levels of brachial-ankle pulse wave velocity and sICAM-1 in the study group were lower than those before treatment(P<0.05).The level of ankle brachial index was slightly higher than that before treatment,but the difference wasn't significant(P>0.05).After 1 months of treatment,the levels of brachial-ankle pulse wave velocity and sICAM-1 in the study group were lower than those in the conventional group(P<0.05).There wasn't significant difference about the levels of ankle brachial index(P>0.05).Conclusion Compared with supragingival scaling,non-surgical periodontal therapy can decrease the level of brachial-ankle pulse wave velocity in patients with moderate to severe periodontitis,and reduce the level of sICAM-1,so it is helpful to reduce the risk of cardiovascular disease.
