脑干出血保守治疗早期强化降压的临床研究

摘　　要：目的探讨脑干出血患者早期强化降压治疗的安全性及有效性。方法选择我院2012—2017年收治的脑干出血患者,分为对照组和观察组。对照组采取常规保守治疗。观察组在常规保守治疗的基础上采取早期强化降压治疗(24h内控制收缩压110~129mmHg)。观察24h后患者血肿扩大情况,并评估3个月后患者的预后。结果对照组患者8例发生血肿增大,发生率34.8%。观察组3例出现血肿增大,发生率15.8%,组间比较差异有统计学意义(P<0.05)。观察组与对照组患者3个月的病死率与致残率无明显差异(P>0.05)。结论脑干出血患者早期强化降压治疗是安全有效的,可降低患者血肿增大的发生率,建议进一步的研究加以证实。Objective To investigate the safety and efficacy of early intensive blood pressure lowering for brain stem hemorrhage patients with conservative treatment.Methods Patients in our hospital with brain stem hemorrhage during 2012 to 2017 were enrolled in this study and divided into two groups.The control group was treated with conventional conservative treatment.The observation group was treated with early intensive blood pressure lowering(keeping SBP at 110 129mmHg in the early 24h)in conservative therapy.The hematomas were rescanned in 24 hours and outcomes for patients were evaluated in 3 months.Results In the control group,the hematoma enlargement occurred in 8 cases,and the incidence was 15.8%.In the observation group 3 cases was detected hematoma increase,and the incidence was 15.8%.The difference was statistically significant(P<0.05),While there was no statistical difference in 3 months outcome,including mortality and morbidity,between the two groups(P>0.05).Conclusion Early intensive blood pressure lowering for patients with brain stem hemorrhage is safe and effective,which can reduce the incidence of hematoma enlargement.Further research is suggested to validate this conclusion.
