摘　　要：目的研究急性大脑中动脉(middle cerebral artery,MCA)脑梗死患者二级侧支循环状态与出院时神经功能改善程度的关系。方法回顾性分析深圳市罗湖区人民医院在发病1周内入院的23例急性MCA梗死患者,采用320排螺旋CT查头CTA,评估二级侧支循环状态、出入院时的NHISS评分和出院时的MRS评分,研究其相关性。结果患者神经功能恢复情况(出院与入院时NIHSS得分差)与二级侧支循环状态得分呈负相关性,出院时MRS得分与二级侧支循环状态得分无显著性相关。结论急性脑梗死患者有好的二级侧支循环出院时,近期神经功能改善较显著,近期总体神经功能状态改善还需延长时间观察。多排CTA在评估侧支循环中具有重要价值。Objective To investigate the relationship between secondary collateral circulation status of acute middle cerebral artery(MCA)cerebral infarction and the improvement of neurological function.Methods We performed retrospective analysis of 23 cases of acute MCA infarction admitted to hospital within a week.Secondary collateral circulation state were assessed by 320 row helical CT.NHISS score at admission and discharge and MRS score at discharge were also evaluated.Results There was a negative correlation between recovery of the patients’neurological function(NIHSS score difference of discharge and admission)and secondary collateral circulation state score;there was no significant correlation between MRS scores at discharge and secondary collateral circulation state score.Conclusion Acute cerebral infarction patients with a good secondary collateral circulation condition had a better improvement in neurological function.But it would take a long time to observe the long term outcomes.Multi row CTA had great values in evaluating collateral circulation.
