II-III 期局部晚期直肠癌术前放化疗的疗效研究

摘　　要：目的分析研究Ⅱ~Ⅲ期直肠癌术前放化疗局部晚期直肠癌术前放化疗的疗效。方法随机抽取该院2012年1月—2014年5月收治的40例局部晚期直肠癌患者,所有的患者均进行卡培他滨单药化疗同步三维适形放疗,放疗5周后所有患者接受手术治疗,观察临床疗效、术后吻合口瘘发生率与、3年内的存活情况与保肛率。结果40例患者均未出现严重不良反应,其主要表现为恶心、呕吐、白细胞下降与放射性直肠炎等,在术前所有患者的肿瘤分析得到显著降期,即原9例T4期患者中,共有2例患者降期为T1期,共有5例患者降期为T2期,1例患者降期为T3期,1例患者仍为T4期;原31例T3期患者中,共有7例患者降期为T1期,共有16例患者降期为T2期,其余8例患者仍为T3期;术后得到了较为准确的病理判断,并且术后仅发生1例口瘘现象,患者在3年内的存活几率达到70.00%、患者的保肛率达到85.00%。结论对局部晚期直肠癌患者进行放化疗,能够有效的降低肿瘤分期,提升治愈率,不良反应可以耐受,对患者具有一定的安全性,值得临床推广。Objective To analyze and research the curative effect of preoperative chemoradiotherapy of rectal cancer in theⅡ~Ⅲstage and local advanced stage.Methods 40 cases of local advanced rectal cancer patients in our hospital from January 2012 to May 2014 were selected and treated with three dimensional-conformal radiotherapy combined with capecitabine,all patients were treated with surgery after five-week chemoradiotherapy,and the clinical curative effect,incidence rate of postoperative anastomotic leakage,3 year survival situation and anal preservation rate were observed.Results There were no severe adverse reactions in 40 cases,mainly featured as nausea,vomiting,neutropenia and radiation proctitis,before surgery,the tumor analysis of all patients obviously decreased,that is to say,in the primary 9 cases,in T4 stage,2 cases decreased to T1 stage,5 cases decreased to T2 stage,1 case decreased to T3 stage,and 1 case was still in T4 stage,and in the primary 31 cases in T3 stage,7 cases decreased to T1 stage,16 cases decreased to T2 stage,and the other 8 cases were still in T3 stage,and the accurate pathological determination showed that there was 1 case with fistula,and the survival probability within 3 years reached 70.00%and anus reservation rate reached 85.00%.Conclusion The chemoradiotherapy of patients with local advanced rectal cancer can effectively reduce the tumor staging,improve the cure rate,and the adverse reactions can be tolerated with safety,and it is worth clinical promotion.
