摘　　要：目的探讨椎管麻醉在高龄骨科手术中的应用效果。方法方便选择2015年1月—2017年1月该院收治的高龄骨科手术患者60例,随机分为观察组和对照组,每组30例,观察组使用椎管麻醉,对照组使用全身麻醉,对比两组患者血流动力学指标、疼痛情况、不良反应。结果 (1)术后12 h及24 h,观察组VAS评分(1.48±0.27)分、(1.24±0.19)分均好于对照组(3.65±0.62)分、(2.57±0.29)分,(P<0.05);(2)观察组不良反应发生率为6.6%,对照组不良反应发生率为26.6%,组间差异有统计学意义(P<0.05);(3)在麻醉的各个时间段,两组的心率及平均动脉压指标均有小幅度的波动,但整体平稳,组间差异无统计学意义(P>0.05)。结论椎管麻醉用于高龄骨科手术麻醉中效果理想,值得推广。Objective To study the application effect of spinal anesthesia in senile orthopedic surgery.Methods 60 cases of senile patients with orthopedic surgery admitted and treated in our hospital from January 2015 to January 2017 were convenient selected and randomly divided into two groups with 30 cases in each,the observation group and the control group respectively adopted the spinal anesthesia and general anesthesia,and the hemodynamics indicators,pain situation and adverse reactions were compared between the two groups.Results The VAS scores at 12 h and 24 h after surgery were better than those in the control group[(1.48±0.27)points,(1.24±0.19)points vs(3.65±0.62)points,(2.57±0.29)points](P<0.05),and the incidence rate of adverse reactions in the observation group and in the control group was respectively 6.6%and 26.6%,and the difference was statistically significant,(P<0.05),and the heart rate and mean arterial pressure of the two groups had a small fluctuation at various time periods of anesthesia,but the whole was stable,and the difference between groups was not statistically significant(P>0.05).Conclusion The effect of spinal anesthesia in senile orthopedic surgery is ideal,which is worth promotion.
