糖尿病视网膜病变激光治疗后干眼治疗临床研究

摘　　要：目的:比较两种人工泪液联合普拉洛芬对糖尿病视网膜病变患者激光治疗后干眼的治疗效果。方法:选取行视网膜激光治疗的糖尿病视网膜患者90例,共90只眼。应用随机表分组法为A组30眼,B组30眼,C组30眼。A组空白对照组,B组经激光治疗后使用普拉洛芬联合玻璃酸钠点眼,C组经激光治疗后使用普拉洛芬联合聚乙二醇点眼。分别对三组患者激光治疗后7、14、28d行干眼问卷调查、基础泪液分泌(SIT)、泪膜破裂时间(BUT)、角膜荧光素染色(FL)结果进行综合分析。结果:OSDI问卷结果显示:治疗后不同时间点的OSDI评分三组差异有统计学意义(H=18.976、H=30.257、H=32.82,P<0.05),B、C组均低于A组(Z=-3.336、-4.539、-4.684;P<0.05;Z=-4.036、-4.684、-4.694;P<0.05),不同时间点B、C组的BUT时间较A组延长(Z=-3.091、-3.341;Z=-4.627、-4.698;Z=-4.776、-4.977;P<0.05)。结论:糖尿病视网膜病变激光治疗后给予含有黏蛋白成分的人工泪液治疗,有利于术后干眼症状与体征的改善。Objective:To compare the curative effect of two different artificial tears with pranoprofen on the laser photocoagulation of diabetic retinopathy.Methods:90 patients(90 eyes)with laser photocoagulation on diabetic retinopathy at Fourth Hospital of Xi'an.These eyes were randomly divided into A group(30 eyes),B group(30 eyes),C group(30eyes).A group was blank group;B group was pranoprofen with sodium hyaluronate eye drop;C group was pranoprofen with polyethylene glycol eye drop.All patients finished a dry eye questionnaire called Ocular Surface Disease Index(OSDI),and Schimmer’s test(SⅠT),break up time(BUT)and fluorescein staining(FL)tests were performed before surgery and at 7,14,28 days after laser photocoagulation.Results:OSDI questionnaire results show:The OSDI scores at different time points after treatment had statistically significant differences among the three groups(H=18.976、H=30.257、H=32.82,P<0.05),B and C groups were lower than A group(Z=-3.336、-4.539、-4.684;P<0.05;Z=-4.036、-4.684、-4.694;P<0.05),BUT at different time points B,C group than the A group extended(Z=-3.091、-3.341;Z=-4.627、-4.698;Z=-4.776、-4.977;P<0.05).Conclusion:The treatment of artificial tears with mucin in the laser photocoagulation on diabetic retinopathy is beneficial to the improvement of postoperative symptoms and signs of dry eye.
