超声弹性成像与MR动态增强显像在乳腺肿瘤良恶性鉴别诊断中的价值

摘　　要：目的:探讨超声弹性成像与MR动态增强显像在乳腺肿瘤良恶性鉴别诊断中的价值。方法:乳腺肿瘤患者98例,病理诊断良性肿瘤45例,恶性肿瘤53例。分别对98例患者进行超声弹性成像(UE)检查、MR动态增强显像检查,观察在不同检查下良、恶性检出结果并与病理对照分析特异率、敏感率及准确率。结果:以病理结果为金标准,UE诊断乳腺恶性肿瘤51例,其中正确诊断41例,误诊12例;UE诊断乳腺良性肿瘤47例,其中正确诊断35例,误诊10例。MR动态增强显像诊断乳腺恶性肿瘤50例,其中正确诊断41例,误诊9例;诊断乳腺良性肿瘤48例,其中正确诊断36例,误诊12例。两者联合诊断乳腺良性肿瘤46例,其中正确诊断41例,误诊5例;诊断恶性肿瘤52例,其中正确诊断48例,误诊4例。UE联合MR动态增强检查下特异率、敏感率、准确率分别为91.11%、90.57%、90.82%明显高于UE的77.78%、77.36%、77.55%及MR动态增强检查的80.00%、77.36%、78.57%(均P<0.05)。结论:UE联合MR动态增强显像特异率、敏感率及准确率均优于单一UE检查或MR动态增强检查,对乳腺肿瘤诊断价值更高,值得推广使用。Objective:To explore the comparison the value of differential diagnosis of the breast tumors with ultrasonic elastography and MR dynamic enhancement phenomenon.Methods:Chosed 98 cases breast neoplasms.According to benign and malignant tumors,45 cases were divided into benign group,and 53 cases in malignant group.The two groups of patients were examined by ultrasonic elastography(UE),MR dynamic contrast enhancement and UE combined with MR dynamic contrast enhancement,two groups of patients were observed in different examination of benign and malignant detection results,pathological comparison,and comparative analysis of specific rate,sensitivity and accuracy rate.Results:UE diagnosis of benign diagnosis group 1~3 points had 35 cases,were significantly higher than the malignant group of 12 cases,4~5 points had 10 cases were significantly lower than the malignant group of 41 cases,the difference was statistically significant(P<0.05);MR enhancement phenomenon of inspection under the benign group morphological diagnosis for round 15 cases,schistose 21 cases was significantly higher than that in malignant group round 5 cases,schistose in 7 cases,and 4 cases of irregular,burrs on the edges of the 5 cases was significantly lower than that in malignant group irregular 18 cases,burrs on the edges of the 23 cases,the difference is statistically significant(P<0.05);UE combined with MR dynamic enhanced examination under the benign group in the diagnosis of a benign tumor cases,41 were significantly higher than those in the malignant group 5 cases.And the diagnosis of malignant tumor cases,4 cases were significantly lower than the malignant group of 48 cases,the difference was statistically significant(P<0.05);UE combined with MR dynamic enhanced the specific rate,rate sensitivity and accuracy were 91.11%,90.57%,90.82%was significantly higher than that of UE 77.78%,77.36%,77.55%and MR dynamic enhanced 80.00%and 77.36%and 78.57%,the difference has statistical significance(P<0.05).Conclusion:The rate of specific,sensitivity and
