子宫颈小细胞神经内分泌癌10例临床病理研究

摘　　要：目的:探讨子宫颈小细胞神经内分泌癌的临床与病理特点。方法:回顾性分析10例子宫颈小细胞神经内分泌癌的临床病理资料,治疗及预后。结果:10例患者平均年龄45.6岁(31~54岁),Ⅰb1期7例,Ⅰb2期1例,Ⅱa期2例。组织学示肿瘤细胞小圆形,胞浆稀少,高核浆比,缺乏核仁。免疫组化示神经内分泌标记物阳性率由高到低依次为Syn(100%,10/10),CgA(90%,9/10),CD56(85.7%,6/7),NSE(62.5%,5/8);广谱CK阳性率(80%,8/10)。10例患者均行腹腔镜下全子宫、双附件切除,盆腔淋巴结清扫术。其中6例术后接受了放化疗。中位随访时间为18.6个月(8~34个月),4例死亡,2例肺转移。结论:子宫颈小细胞神经内分泌癌恶性度高,具有独特的组织学与免疫表型特点,易早期发生转移,预后差,组织病理学联合免疫组化检测可提高其诊断的准确率,并在鉴别诊断中起重要作用。Objective:To investigate the clinicopathological characteristics of cervical small cell cancer.Methods:To analyse 10 cases of cervical small cell cancer's clinical and pathological data,treatment and prognosis.Results:The average age was 45.6 years(31 to 54 years old),Ⅰb1 7 cases,Ⅰb2 1 case,Ⅱa 2 cases.Histologically,the neoplastic cells appeared small,round in solid sheets with scant cytoplasm,hyperchchromatic nuclei and sbsence of nucleoli.Immunohistochemical studies showed positive rate of neuroendocrine markers for Syn(100%,10/10),CgA(80%,8/10),CD56(85.7%,6/7),NSE(62.5%,5/8),Positive rate for pan-CK was(80%,8/10).10 patients underwent extensive hysterectomy with bilateral oophorectomy and pelvic lymphadenectomy.6 patients received postoperative chemotherapy.The median follow-up time was 18.6 months(8 to 34 months),4 cases of death,2 cases of pulmonary metastasis.Conclusion:Cervical small cell cancer has the characteristics of highly malignant degree,early distant metastasis,and poor prognosis.Histopathological joint immunohistochemical detection can improve the accuracy of diagnosis and play an important role in differential diagnosis.
