超声造影时间-强度曲线联合血清学检测在肝脏肿瘤良恶性鉴别诊断中的价值

摘　　要：目的探究超声造影时间-强度曲线(TIC)与血清学检查在肝脏肿瘤良恶性鉴别诊断中的价值。方法将本院的肝脏肿瘤患者纳入研究,行血清学检测和超声造影TIC定量检测,并根据病理检测结果将其分为恶性肿瘤组和良性肿瘤组,比较分析两组之间所测量指标的差异,并对有统计学差异的指标进行ROC曲线的构建和比较。结果最终纳入患者205例,恶性肿瘤组89例,良性肿瘤组116例。恶性肿瘤组的血清学指标甲脂蛋白含量和超声造影峰值强高于良性肿瘤组,差异有统计学意义(P<0.05)。ROC曲线结果提示,AFP含量,超声造影峰值强度的曲线面积分别为82.7%,78.8%,联合预测ROC曲线下面积为91.8%。结论血清学指标和超声造影对肝脏良恶性肿瘤分析均有一定的诊断效能,而两者联合诊断的预测曲线则提示,血清学与超声造影联合诊断效能明显优于二者的单独诊断效能,有望成为临床上一种无创精确简便的诊断方法。To evaluate the clinical value of contrast-enhanced ultrasound time-intensity curve(TIC)technology combined with serologic test in differential diagnosis of the liver benign and malignant tumors.The patients with the liver tumor in our hospital were involved in this study and received serologic test and contrast-enhanced ultrasound TIC examination.According to the pathological test results,the patients were divided into the malignant tumor group and the benign tumor group.Serological tests and contrast-enhanced ultrasound indexes were compared between the two groups.Then,the ROC curve analysis was performed to compare their diagnostic value.A total of 205 patients were involved,in which 89 had malignant tumor and 116 had benign tumor.The serum levels of AFP and the maximum of intensity of the malignant tumor group were higher than these of the benign tumor group(P<0.05).The result of ROC curve showed that the AUC of AFP and the maximum of intensity were 82.7%and 78.8%,respectively.The AUC of combination of two techniques was 91.8%.Serological test and ultrasonic imaging all have certain diagnostic value for benign and malignant liver tumors.However,combination of the two methods is better than the two methods alone.This approach is expected to become an accurate noninvasive clinical method.
