摘 要:目的研究散发性结直肠癌患者错配修复蛋白(MMRP)表达情况以及微卫星不稳定性(MSI)与视网膜母细胞瘤(Rb)蛋白表达的相关性。方法选取2013年1月—2016年1月的手术切除的散发性结直肠癌标本60例。采用免疫组织化学法以及组织芯片检测MMRP和Rb蛋白的表达情况,将4种MMRP(MLH1、PMS2、MSH2和MSH6)中1种以上表达阴性定为MSI。分析结直肠癌患者的MMRP表达情况,MSI和Rb蛋白与临床病理特征的关系以及MSI和Rb蛋白表达的相关性。结果 MLH1阳性54例,阴性6例;PMS2阳性52例,阴性8例;MSH2阳性59例,阴性1例;MSH6阳性58例,阴性2例;MSI患者10例(16.67%)。Rb蛋白阴性表达25例(41.67%),阳性表达35例(58.33%)。MSI与年龄、肿瘤部位、Dukes分期、肿瘤直径、淋巴结转移、组织类型存在相关性(P<0.05),Rb蛋白表达与组织类型、远处转移、浸润深度、大体分型存在相关性(P<0.05)。MSI与Rb蛋白表达呈正相关(r=0.427,P<0.001)。结论散发性结直肠癌患者存在MMRP缺失,并且MLH1、PMS2缺失表达较MSH2和MSH6多见。MSI和Rb蛋白与散发性结直肠临床病理特征关系密切,且二者呈正相关,可能参与散发性结直肠癌的发生、发展过程。Objective To investigate correlations between mismatch repair protein(MMRP)expression and microsatellite instability(MSI)with expression of retinoblastoma(Rb)protein in patients with sporadic colorectal cancer.M Methods A total of 60 samples of sporadic colorectal cancer by surgical resection during January 2013 and January 2016 were selected.Expressions of MMRP and Rb proteins were detected by immunohistochemistry and tissue microarray detections,and more than 1 kind of negative expression in 4 kinds of MMRP(MLH1,PMS2,MSH2 and MSH6)were defined as MSI.MMRP expressions in patients with colorectal cancer were analyzed,and correlations between MSI and Rb protein expressions with clinicopathological features,and expressions of MSI and Rb protein were analyzed.Results There were 54 cases of positive MLH1 and 6 cases of negative;there were 52 cases of positive PMS2 and 8 cases of negative;there were 59 cases of positive MSH2 and 1 case of negative;there were 58 cases of positive MSH6 and 2 cases of negative.MSI was found in 10 cases(16.67%);negative expression of Rb protein was found in 25 cases(41.67%),and positive expression was found in 35 cases(58.33%).MSI expression was correlated with age,tumor location,Dukes staging,tumor diameter,lymph node metastasis and tissue types(P<0.05),while expression of Rb protein was correlated with tissue types,distant metastasis,infiltrating depth and general typing(P<0.05).MSI expression was positively correlated with expression of Rb protein(r=0.427,P<0.001).Conclusion Mismatch repair protein deletion can be found,and deletion expressions of MLH1 and PMS2 are more common than expressions of MSH2 and MSH6 in patients with sporadic colorectal cancer.MSI and Rb proteins are closely related to clinicopathological features of sporadic colorectal cancer,and they are positively correlated,and they may be involved in pathogenesy and development of sporadic colorectal cancer.
