摘 要:目的研究早产儿脐带血维生素D[25-(OH)D]水平与婴儿期生长发育的关系。方法临床纳入200例2015年9月~2016年9月期间在我院出生的早产儿作为研究组,根据胎龄情况分组,胎龄≤32周的50例早产儿作为研究1组,胎龄>32周的150例早产儿作为研究2组。另选取同期在我院出生的100例正常足月儿作为对照组。检测所有研究胎儿脐带血25-(OH)D水平。对早产儿及足月儿均进行补充维生素D治疗,不同组补充剂量不同。观察三组胎儿治疗后生长发育情况。结果三组胎儿治疗前25-(OH)D水平有明显差异,治疗组高于研究2组,高于研究1组(均P<0.05)。按照不同组和维生素D3缺乏情况进行维生素D3补充治疗,治疗后发现三组胎儿25-(OH)D水平差异无统计学意义(P>0.05)。治疗前三组胎儿体重、身长以及头围均有差异,治疗组明显大于研究2组,大于研究1组(P<0.05)。治疗后随访1年,三组胎儿体重、身长以及头围均差异无统计学意义(P>0.05)。结论早产儿相对于足月儿来说生长发育相对较快,针对性对患儿进行维生素D补充,可有效解决早产儿生长发育过快导致的维生素D不足问题,值得临床应用及推广。Objective To study the relationship between vitamin D[25-(OH)D]levels in umbilical cord blood of premature infants and their growth and development in infancy.Methods A total of 200 premature infants born in our hospital from September 2015 to September 2016 were enrolled as study group,and were grouped according to gestational age.Fifty premature infants with gestational age≤32 weeks were selected as study group 1.One hundred and fifty premature infants with gestational age>32 weeks were selected as study group 2.One hundred cases of normal full-term infants born in our hospital during the same period were selected as control group.The levels of 25-(OH)D in all umbilical cord blood were detected.All the premature infants and full-term infants were given vitamin D supplementation.Different groups had different dosages.The growth and development of three groups of fetuses after treatment were observed.Results There was significant difference in 25-(OH)D level between three groups before fetal treatment.The treatment group was higher than study group 2 and study group 1(all P<0.05).Vitamin D3 supplementation was administered according to different groups and vitamin D3 deficiency.After treatment,there was no significant difference in the level of 25-(OH)D between the three groups(P>0.05).The fetal weight,body length and head circumference of the three groups were different before treatment,The treatment group was significantly larger than study group 2 and study group 1(P<0.05).After 1 year followup,there was no significant difference in fetal weight,body length and head circumference between the three groups(P>0.05).Conclusion The growth and development of preterm infants are relatively fast relative to the full-term infants.Targeted vitamin D supplementation for children can effectively solve the problem of vitamin D deficiency caused by excessive growth and development of preterm infants,which is worthy of clinical application and promotion.
