C臂CT引导下经皮肺穿刺活检在肺部小结节诊断中的应用分析

摘　　要：目的探讨C臂CT引导下经皮肺穿刺活检在肺部小结节诊断中的应用价值。方法选取2015年7月~2017年10月我院收治的107例肺部小结节(病灶直径为1.0~3.0cm)患者为主要研究对象,所有患者均在我院接受C臂CT引导下经皮肺穿刺活检,在经皮肺穿刺活检前、穿刺中以及穿刺后均采用C臂CT机进行辅助扫描,对诊断结果进行回顾性分析。结果 C臂CT引导下经皮肺穿刺活检诊断肺部小结节的灵敏度和特异度分别为88.89%、100%,未发现假阳性病例。穿刺后有10例患者发生气胸,其中3例实施闭式引流术治疗,有3例患者术后发生咯血,未经特殊处理后患者自行痊愈。结论在肺部小结节的临床诊断上,采用C臂CT引导下经皮肺穿刺活检进行检查和诊断,可提高肺部小结节的穿刺阳性率,同时可以及时发现并处理术后并发症,提高检查与诊断的安全性,值得在临床上进一步推广和应用。Objective To explore the application value of C arm CT guided percutaneous lung biopsy in the diagnosis of small pulmonary nodules.Methods From July 2015 to October 2017,107 patients with small pulmonary nodules(diameter of the lesion was 1.0-3.0cm)admitted to our hospital were selected as the main study subjects.All patients received C arm CT guided percutaneous pulmonary biopsy in our hospital.Auxiliary scanning in percutaneous lung biopsy before and after puncture,puncture using C arm ct,the diagnostic results were retrospectively analyzed.Results The sensitivity and specificity of C-arm CT guided percutaneous pulmonary biopsy in diagnosing pulmonary nodules were 88.89%and 100%,respectively.No false-positive cases were found.Pneumothorax occurred in 10 patients after the puncture.Among them,3 cases were treated by closed drainage,and 3 cases had hemoptysis after operation.Patients were cured without special treatment.Conclusion In the clinical diagnosis of small pulmonary nodules,C arm CT guided percutaneous lung biopsy can improve the puncture positive rate of small pulmonary nodules,and can be found and processed in time to improve the safety of examination and diagnosis.It is worthy of further popularization and application in clinic.
