摘　　要：目的探讨嗜酸粒细胞胃肠炎(eosinophilic gastroenteritis,EG)的临床特点,提高临床医师对该病的认识及诊疗水平。方法收集2011年6月至2016年7月河南省人民医院确诊的31例EG患者的临床资料,对其进行回顾性分析并分组比较。结果 31例EG患者中,12例(38.7%)有过敏史或过敏性疾病;常见临床表现为腹痛、腹胀、恶心、呕吐、腹泻,以腹痛(87.1%)最常见;临床分型以黏膜型为主(占83.9%);23例(74.2%)伴外周血嗜酸粒细胞(eosinophils,Eos)增多,腹水沉渣病理见大量Eos;内镜下表现无特异性,组织病理学检查见黏膜慢性炎伴大量Eos浸润,以十二指肠、回肠多见,且检出率高。激素治疗有效,且外周血Eos下降显著(P<0.05)。结论 EG是一种变态反应性疾病,其临床表现及内镜下表现均无特异性,确诊依据组织或腹水病理学检查见大量Eos,予抗过敏或激素治疗可明显缓解症状。Objective To explore the clinical characteristics of eosinophilic gastroenteritis(EG),and to improve clinicians’awareness and diagnosis and treatment of this disease.Methods Clinical data of 31 patients with EG confirmed by He’nan Province People’s Hospital from Jun.2011 to Jul.2016 were collected and analyzed retrospectively.Results In the 31 cases of EG,the clinical mainfestations with EG were abdominal pain,abdominal distension,nausea,vomiting,diarrhea,and abdominal pain(87.1%).The mucosal type(83.9%)was common clinical manifestation with EG;the eosinophils(Eos)increased were found in 23 cases(74.2%),a large number of Eos were detected in ascites pathology;the endoscopic manifestations had no specificity,the mucosal chronic inflammation was detected by histopathological examination.A large number of Eos infiltration were detected by histopathological examination.The duodenum and ileum were more common and had higher detection rate.Compared with hormone therapy group,the peripheral blood Eos were decreased significantly in hormone therapy group(P<0.05),and the hormone therapy was effective to EG.Conclusion EG is an allergic disease,its clinical manifestations and endoscopic manifestations are not specific,according to the diagnosis of pathological examination of the organization or ascites see a lot of Eos,to anti-allergy or hormone therapy can relieve symptoms.
