摘 要:[目的]了解2014年广东省二级、三级综合医院护理工作环境现状,并与2009年情况进行比较。[方法]采用护士一般资料调查表和护理工作环境量表(PES-NWI)对广东省23所二级、三级综合医院133个病区2 066名护士进行抽样调查,并与2009年《全国医院护理人力资源研究》中广东省的有关数据进行比较。[结果]2014年PES-NWI量表5个维度得分居后2位的分别是"人力物力充足性"和"护士参与医院事务",分别为2.85分和2.88分;多水平模型分析结果显示,2014年护士对于"人力物力充足性""护士参与医院事务"和"医护合作关系"3个维度评价下降(P<0.05)。[结论]在控制了医院结构性因素及护士个人因素的影响后,2014年广东省二级、三级医院护士对护理工作环境的评价较2009年水平无显著提高,尤其是人力物力充足性、护士参与医院事务和医护合作关系亟待改进。Objective:To understand the status quo of nursing work environment in the second and third grade general hospitals in Guangdong province in 2014,and to compare with the situation in 2009.Methods:The nurses general information questionnaire and Practice Environment Scale of Nursing Work Index(PES NWI)were used to conduct a sampling survey of 2066 nurses in 133 wards in 23 general hospitals in Guangdong province and compared with the relevant data of the national hospital nursing human resources research Guangdong province in 2009.Results:The scores of last two of the five dimensions of the PES NWI scale in 2014 were“manpower and material adequacy”and“nurse participation in hospital affairs”,which were 2.85 and 2.88 respectively.The results of multilevel model analysis showed that in 2014,there was a decline in the evaluation of 3 dimensions,including manpower and material adequacy,nurse participation in hospital affairs and cooperative relationship between medical care and nursing(P<0.05).Conclusions:After controlling the influence of hospital structural factors and the personal factors of the nurses,in 2014 the evaluation of nurses on working environment in second grade and third grade hospitals in Guangdong had no significant improvement compared with that in 2009.In particular,manpower and material adequacy,nurse participation in hospital affairs and doctor nurse cooperative relationship needed to be improved.
