后腹腔镜下离断式肾盂成形术治疗小儿肾盂输尿管连接部狭窄的效果评价

摘　　要：目的探讨评价后腹腔镜下离断式肾盂成形术治疗小儿肾孟输尿管连接部狭窄的临床效果。方法选择2015年10月~2016年10月在我院接受肾孟输尿管连接部狭窄82例患儿作为研究对象,按随机数字表法分为研究组和参照组,每组各41例。参照组患儿行开放性离断式肾盂成形术,研究组患儿则采取后腹腔镜下离断式肾盂成形术进行治疗。比较两组患儿的手术时间、肠道功能恢复时间、住院时间以及术中出血量等情况,并随访比较两组的手术成功率、术后并发症情况和肾积水的复发情况。结果研究组患儿的手术时间、肠道功能恢复时间、住院时间以及术中出血量均明显低于参照组,差异均有统计学意义(P<0.05),研究组患儿的手术成功率、术后并发症发生率以及肾积水复发率分别为95.1%、7.3%、2.4%,与参照组比较,差异无统计学意义(P>0.05)。结论运用后腹腔镜下离断式肾盂成形术治疗小儿肾孟输尿管连接部狭窄手术时间短,患者术后恢复快,不失为一种安全有效的手术方法,值得临床进行广泛应用。objective To evaluate the clinical effect of retroperitoneal disconnection pyeloplasty in the treatment of ureteral junction stenosis in children.Methods From October 2015 to October 2016,82 cases of ureteral junction stricture were studied in our hospital.They were randomly divided into study group and reference group according to random number table method,41 cases in each group.The patients in the control group were treated with open dissected pyeloplasty,and those in the study group were treated by retroperitoneal laparoscopic dissecting pyeloplasty.The operative time,intestinal function recovery time,hospitalization time and intraoperative blood loss were compared between the two groups.And the success rate,postoperative complications and recurrence of hydronephrosis were compared between the two groups.Results The operative time,intestinal function recovery time,hospitalization time and intraoperative blood loss in the study group were significantly lower than those in the control group(P<0.05).The success rate of operation,the incidence of postoperative complications and the recurrence rate of hydronephrosis in the study group were 95.1%,7.3%and 2.4%respectively,there was no significant difference compared with the control group(P>0.05).Conclusion Laparoscopic pyeloplasty for the treatment of ureteral junction stricture in children was performed in a short time,and the patients recovered quickly after operation.It is a safe and effective surgical treatment method,which is worthy of extensive clinical application.
