摘　　要：目的探讨WHO Ⅲ级脑膜瘤术后无进展生存期(progression free survival,PFS)、总生存期(overal survival,OS)的影响因素。方法收集2010年3月至2017年3月于天坛医院神经外科经手术治疗、术后病理确诊且有完整随访资料的60例WHO Ⅲ级脑膜瘤患者的临床资料。应用单因素分析、多因素Cox模型回顾性分析60例WHO Ⅲ级脑膜瘤患者的无进展生存期、总生存期的影响因素,包括:年龄、性别、肿瘤大小、部位、切除程度、手术史、放射治疗、病理类型等。结果 WHO Ⅲ级脑膜瘤病人的PFS 18(9,34)个月,OS 27(17,44)个月。单因素分析表明年龄(P=0.048)、肿瘤部位(P=0.043)、手术史(P=0.007)、放射治疗(P<0.001)和病理类型(P=0.048)为PFS的影响因素,而多因素分析进一步证实放射治疗(P<0.001)为PFS的影响因素,未发现OS的影响因素。结论 WHO Ⅲ级脑膜瘤患者易复发、生存期短,而放射治疗有利于延长PFS。Objective To analyze the prognostic factors of progression free survival(PFS)and overall survival(OS)in postoperative patients with WHO grade III meningioma.Methods Retrospective analysis of the progression free survival and overall survival of 60 postoperative patients diagnosed with WHO III meningioma in the Department of Neurosurgery of Beijing Tiantan Hospital from March 2010 to March 2017.Different prognostic factors including age,gender,tumor volume,tumor location,extent of resection,surgery history,radiotherapy and pathology.Prognostic factors were assessed by Kaplan-Meier analysis and cox proportional hazard regression model.Results The median progression free survival was 18(9,34)months.Univariate analysis showed that age(P=0.048),tumor location(P=0.043)and operation history(P=0.007),radiotherapy(P<0.001)and pathological type(P=0.048)had a significant impact on PFS.Multivariate survival analysis showed that radiotherapy(P<0.001)had significant effects on PFS.Univariate and multivariate survival analysis showed no factor had significant impact on OS(P>0.05).Conclusion WHO grade III meningiomas tend to relapse easily and have a shorter survival time and radiation therapy is beneficial to prolong PFS.
