改良VAD方案治疗老年多发性骨髓瘤的临床疗效及安全性分析

摘　　要：目的研究改良VAD方案治疗老年多发性骨髓瘤的临床疗效及不良反应发生情况。方法选择2012年3月~2017年3月在本院进行诊治的老年多发性骨髓瘤患者24例,所有患者均采取改良VAD方案进行治疗,观察24例患者的临床治疗效果和不良反应。结果 24例患者中SCR 2例,CR 7例,VGPR 3例,PR 4例,SD 3例,PD 5例,无CR后复发,总有效率为79%。无治疗相关死亡;18例出现周围神经病变;17例患者出现Ⅰ-Ⅱ度骨髓抑制,7例出现Ⅲ度骨髓抑制。结论改良VAD方案对老年多发性骨髓瘤具有显著的临床效果,且不良反应可耐受,安全性较好。Objective To study the clinical effect and safety of modified VAD regimen in the treatment of elderly multiple myeloma.Methods From March 2012 to March 2017,24 elderly patients with multiple myeloma were treated in the hospital.All the patients were treated with modified VAD regimen,and the clinical effect and adverse reactions of 24 patients were observed.Results Between the 24 patients,2 cases were SCR,7 cases were CR,3 cases were VGPR,4 cases were PR,3 cases were SD,5 cases were PD,with no recurrence after CR.The total effective rate was 79%.There were no treatment-related deaths.There were 18 cases of peripheral neuropathy,17 cases ofⅠ-Ⅱdegree myelosuppression,and 7 cases ofⅢdegree myelosuppression.Conclusion The modified VAD has significant clinical effect on elderly patients with multiple myeloma. The adverse reactions can be tolerated and the safety is better.
